Introduction. Pediatric traumatology is a complex field that requires a comprehensive understanding of physeal development, remodeling potential, and the ossification process in order to ensure appropriate patient treatment. The objective of this study was to assess the willingness of practicing physicians to participate in a telemedicine collaboration aimed at enhancing the exchange between the outpatient and inpatient sectors and promoting the digitalization of the pediatric sector. This is in response to the growing significance of digitalization in the medical field. Methods. A survey consisting of 15 items was sent to 800 practicing trauma surgeons, pediatric surgeons, and pediatricians within a 100 km radius of Hamburg, Germany. The survey included questions about the respondents’ professional experience and telemedicine experience, as well as inquiries about possible telemedicine collaborations. Results. The response rate was 19.3%. Less than half of the participants already used telemedicine in daily practice. In general, 75% of respondents expressed an interest in collaborating with the inpatient sector. The most common reasons for hospital referral were the need for surgery, inadequate treatment of children in practice and co-assessment. The majority were in favor of flexible communication, either via video telephony, imaging applications like or messaging applications. Conclusions. The study revealed a high level of interest in telemedicine collaboration. Information exchange should be tailored to individual needs, with practitioners requiring a versatile and personalized approach that includes imaging. Strict enforcement of data protection regulations is essential. Further research is needed to evaluate the effectiveness of telemedicine collaboration in the treatment of pediatric trauma in both hospital and outpatient settings. Full article

Deep brain stimulation (DBS) for Parkinson’s disease (PD) often necessitates frequent clinic visits for stimulation program optimization, with limited experience in remote patient management. Due to the resource-intensive nature of these procedures, we investigated a way to simplify stimulation optimization for these patients that allows for the continuous monitoring of symptoms while also reducing patient burden and travel distances. To this end, we prospectively recruited ten patients treated with DBS for PD to evaluate the feasibility of telemedicinal optimization in a home-based setting. Patients recorded daily videos of a modified Unified Parkinson’s Disease Rating Scale (UPDRS) III, which experienced DBS physicians located at the clinic assessed to provide instructions on adjusting stimulation settings using a handheld programmer with previously set programs as well as patient amplitude control. This study concluded with significant improvements in participants’ motor status as measured by the UPDRS-III (p = 0.0313) compared to baseline values. These findings suggest that remote video-guided optimization of DBS settings is feasible and may enhance motor outcomes for patients. Full article

Telepharmacy education should be incorporated into the curricula due to its beneficial effects on students, providing pharmacy services during their practice. Therefore, this study aims to explore the knowledge and perceptions of pharmacy students regarding the integration of telepharmacy services into their education curriculum in Saudi Arabia. A cross-sectional study was conducted using an online survey from 1 June to 30 September 2023, among pharmacy students from five universities in Saudi Arabia. The questionnaire was divided into three sections, and descriptive statistics and a generalized linear model were used for analysis. A total of 523 pharmacy students participated. Approximately half of the students were aware of telepharmacy, and only one-quarter had studied it as part of their curriculum. Students believed that telepharmacy education should cover communication, reimbursement, and training for virtual patient interactions. There was a significant positive correlation (p < 0.0001) between the knowledge and perception scores. In addition, students who had heard about telepharmacy before and those with a “somewhat” confidence level showed a significantly positive correlation with knowledge scores (p = 0.01). In conclusion, perception scores, students who had heard of telepharmacy, and those with a “somewhat” confidence level were all positively correlated with pharmacy students’ understanding of telepharmacy. This study underscores the importance of integrating telepharmacy education and practical training into pharmacy curricula to prepare future pharmacists for the evolving healthcare landscape. The COVID-19 pandemic has significantly accelerated the adoption of telemedicine and digital health technologies, revealing their immense potential in managing cancer patients effectively. This article explores the impact of recent technological developments and widened consumer perspectives on personalised healthcare and patient awareness, particularly in oncology. Smartphones and wearable devices have become integral to daily life, promoting healthy lifestyles and supporting cancer patients through remote monitoring and health management. The widespread use of these devices presents an unprecedented opportunity to transform clinical trials and patient care by offering convenient and accessible means of collecting health data continuously and non-invasively. However, to fully harness their potential, it is crucial to establish standardised methods for measuring patient metrics to ensure data reliability and validity. This article also addresses the challenges of integrating these technologies into clinical practice, such as cost, patient and professional reluctance, and technological oversaturation. It emphasises the need for continuous innovation, the development of robust digital infrastructures, and the importance of fostering a supportive environment to integrate these advancements permanently. Ultimately, the convergence of technological innovation and personalised healthcare promises to enhance patient outcomes, improve quality of life, and revolutionise cancer management in the post-COVID era. The adoption of telemedicine among the elderly is vital due to their unique healthcare needs and growing engagement with technology. This study explores the factors influencing their adoption behaviors, identifying both facilitating and inhibiting elements. While previous research has examined these factors, few have empirically assessed the simultaneous influence of barriers and enablers using a sample of elderly individuals. Using behavioral reasoning theory (BRT), this research investigates telehealth adoption behaviors of the elderly in India. A conceptual model incorporates both “reasons for” and “reasons against” adopting telehealth, capturing the nuanced dynamics of adoption behaviors. Data from 375 elderly individuals were collected to validate the model through structural equation modeling. The findings reveal that openness to change significantly enhances attitudes towards telehealth and “reasons for” adoption, influencing behaviors. This research contributes to the healthcare ecosystem by improving the understanding of telehealth adoption among the elderly. It validates the impact of openness to change alongside reasons for and against adoption, refining the understanding of behavior. By addressing impediments and leveraging facilitators, this study suggests strategies to maximize telehealth usage among the elderly, particularly those who are isolated, improving their access to medical services. This study provides an overview of trends and evolution in International Organization for Standardization (ISO) implementation in healthcare over the past three decades, useful for researchers, practitioners, and policymakers in the sector. As an additional contribution, we propose the NU-index, which allows the evaluation of authors’ scientific output in a comprehensive way that will be valuable for researchers, institutions, and funding agencies seeking to assess researcher performance more accurately and understandably. A bibliometric analysis of 476 scientific articles from the Scopus Database was conducted, examining the scientific output, key authors, high-impact journals, and frequent keywords using VOSviewer, Bibliometrix library, and statistical analysis. Results reveal a significant increase in publications on ISO standards in healthcare since 2009, particularly in areas such as telemedicine, artificial intelligence, and information security. The emergence of ISO 11073 (health informatics) and ISO 13606 (electronic health records) highlights a growing trend towards information and communication technologies. Despite reported implementation barriers, such as lack of top management commitment and associated costs, adoption of these standards is increasing due to the demand for high-quality healthcare services and the need to comply with regulatory requirements. Background: The need for telepractice and digital treatment has increased due to issues this revision.regarding medical access and the COVID-19 pandemic. However, in many countries, telepractice is rarely performed. The purpose of this pilot study was to describe the detailed process of telepractice in patients with hyperfunctional voice disorder and investigate its effects. Methods: The three subjects who were enrolled in this pilot study had hyperfunctional voice disorders. The evaluation was performed face to face. Auditory perceptual evaluation, acoustic evaluation, aerodynamic evaluation, patient self-evaluation, and interviews were conducted. Treatment was delivered by telepractice using a smartphone application. Results: In quantitative analysis of auditory perceptual evaluation, acoustic evaluation, aerodynamic evaluation, and patient self-evaluation, all subjects showed improved voice after treatment. In-depth analysis of telepractice was performed through the interview. Conclusions: Telepractice was effective in patients with voice disorders, and the patients were satisfied with this approach. In addition to this pilot study, further large-scale studies are required, but telemedicine may improve treatment outcomes and patient satisfaction in cases where medical access is limited or during outbreaks of respiratory infections like COVID-19. Cytomegalovirus (CMV) retinitis is the most common eye disease associated with CMV infection in immunocompromised individuals. The CMVR may initially be asymptomatic; however, relatively mild vitreous inflammation at the onset may be an important differential point from other diseases in HIV patients. Fundus photography, CD4 T-cell count, and telemedicine could be used to screen and monitor the high-risk population, particularly in resource-limited regions. Retinitis generally starts in the peripheral retina and advances toward the posterior pole, which could develop to the characteristic “pizza pie” appearance marked by central retinal necrosis and intraretinal hemorrhage. CMVR causes vision loss if left untreated, and early antiviral therapy significantly reduces the risk of vision loss. Alongside traditional antiviral treatments, immunotherapies including CMV-specific adoptive T-cell therapy and CMV immunoglobulin (CMVIG) are emerging as promising treatment options due to their favorable tolerability and reduced mortality. This review comprehensively examines CMV retinitis, encompassing the clinical features, differential diagnosis, laboratory tests, and updated treatment strategies to inform clinical management. (1) Background: Ocular emergencies account for 1.5–3% of emergency department (ED) visits and require urgent diagnosis to prevent serious complications. Ultrasonography is a crucial, non-invasive diagnostic tool for these conditions but traditionally lacks portability and integration with modern electronic smart devices. The purpose of this study was to assess the accuracy and performance of a new handheld ultrasound device in comparison to a conventional cart-based sonographic machine in patients attending to the ED for vitreo-retinal diseases. (2) Methods: three specialists in ophthalmology, with at least 4-year experience in vitreo-retinal diseases and eye ultrasound, evaluated images of 50 eyes with both portable and traditional ultrasound probes. Each specialist made the diagnosis based on the images captured with both probes and then rated their overall image quality and confidence of diagnosis with a five-point Likert scale. The concordance of diagnosis between the two probes was evaluated. (3) Results: The sample comprised 42 patients. Twenty (40%) healthy eyes and thirty eyes with the following vitreo-retinal interface conditions were examined: 12 retinal detachment (24%), 8 vitreous hemorrhage (16%), and 10 posterior vitreous detachment (20%). The overall accuracy of the two devices appeared to be comparable (70.7% vs. 69.3%). The Butterfly iQ+ probe showed similar sensitivity in retinal detachment diagnosis (91.7% vs. 94.4% of the Accutome B-scan Pro), while it showed poor performance in diagnosing posterior vitreous detachment (sensitivity = 27.2%); (4) Conclusions: The Butterfly iQ+ device demonstrated high sensitivity in the diagnosis of retinal detachment. Significant adjustments are still needed to improve the resolution of the vitreous body. Full article

Background: Obstructive sleep apnea (OSA) is a common disorder associated with major cardiovascular and neurocognitive sequelae. Continuous positive airway pressure (CPAP) is the standard treatment for OSA. The aim of this study was to investigate the prevalence and associations of long-term CPAP adherence in newly diagnosed OSA patients. Methods: We enrolled patients who were diagnosed with OSA during the COVID-19 pandemic. Adherence was defined as CPAP use ≥4 h per night on ≥70% of nights over 30 consecutive days. Patient demographics were retrieved from medical records, and CPAP adherence at 6 months and 1 year after initiation was monitored. Results: Overall, 107 patients were included in the analysis. A number of 73 (68%) and 63 (59%) patients were adherent to CPAP treatment at 6 months and 12 months accordingly. Among the factors examined and analyzed (age, gender, BMI, Apnea–Hypopnea Index (AHI)), no significant correlation was found. Further analysis revealed the potential role of comorbidities. CPAP compliance at 6 months was shown to be associated with better CPAP adherence at 12 months. Conclusions: CPAP adherence at 6 months is correlated to long-term adherence to treatment. Therefore, early close follow-up is important. Further prospective studies are needed to identify other potential predictors. Full article

Musculoskeletal disorders are common among older adults, affecting mobility and quality of life. Effective rehabilitation is essential, but the implementation of programs faces challenges. Traditional methods often necessitate in-person assessments, which can be difficult for older adults with mobility limitations. Telerehabilitation offers a solution, bringing therapy closer to patients. However, the accurate remote monitoring of health and performance remains a challenge. This study addresses this gap by developing and validating the System for Tracking and Evaluating Performance (STEP). STEP is a hardware-software system that automates physical performance tests, eliminating the need for constant expert supervision. The system focuses on three standard tests: the Six-Minute Walking Test (6MWT), the Ten-Meter Walking Test (10MWT), and the 30-s Sit-to-Stand Test (30STS). Validation compared results from the STEP app with in-person assessments by physicians for patients undergoing rehabilitation after knee or hip arthroplasty. The study found strong positive correlations between the app’s results and the physicians’ assessments for all tests. These findings demonstrate the STEP system’s potential as a reliable tool for remote physical performance assessment. Further research is needed to explore its integration into clinical practice and cost-effectiveness in reducing the need for operator assistance in monitoring patients with physical limitations. Background/Objectives: The accurate quantification of ground-glass opacities (GGOs) and consolidation volumes has prognostic value in COVID-19 patients. Nevertheless, the accurate manual quantification of the corresponding volumes remains a time-consuming task. Deep learning (DL) has demonstrated good performance in the segmentation of normal lung parenchyma and COVID-19 pneumonia. We introduce a Human-in-the-Loop (HITL) strategy for the segmentation of normal lung parenchyma and COVID-19 pneumonia that is both time efficient and quality effective. Furthermore, we propose a Gaussian Mixture Model (GMM) to classify GGO and consolidation based on a probabilistic characterization and case-sensitive thresholds. Methods: A total of 65 Computed Tomography (CT) scans from 64 patients, acquired between March 2020 and June 2021, were randomly selected. We pretrained a 3D-UNet with an international dataset and implemented a HITL strategy to refine the local dataset with delineations by teams of medical interns, radiology residents, and radiologists. Following each HITL cycle, 3D-UNet was re-trained until the Dice Similarity Coefficients (DSCs) reached the quality criteria set by radiologists (DSC = 0.95/0.8 for the normal lung parenchyma/COVID-19 pneumonia). For the probabilistic characterization, a Gaussian Mixture Model (GMM) was fitted to the Hounsfield Units (HUs) of voxels from the CT scans of patients with COVID-19 pneumonia on the assumption that two distinct populations were superimposed: one for GGO and one for consolidation. Results: Manual delineation of the normal lung parenchyma and COVID-19 pneumonia was performed by seven teams on 65 CT scans from 64 patients (56 ± 16 years old (μ ± σ), 46 males, 62 with reported symptoms). Automated lung/COVID-19 pneumonia segmentation with a DSC > 0.96/0.81 was achieved after three HITL cycles. The HITL strategy improved the DSC by 0.2 and 0.5 for the normal lung parenchyma and COVID-19 pneumonia segmentation, respectively. The distribution of the patient-specific thresholds derived from the GMM yielded a mean of −528.4 ± 99.5 HU (μ ± σ), which is below most of the reported fixed HU thresholds. Conclusions: The HITL strategy allowed for fast and effective annotations, thereby enhancing the quality of segmentation for a local CT dataset. Probabilistic characterization of COVID-19 pneumonia by the GMM enabled patient-specific segmentation of GGO and consolidation. The combination of both approaches is essential to gain confidence in DL approaches in our local environment. The patient-specific probabilistic approach, when combined with the automatic quantification of COVID-19 imaging findings, enhances the understanding of GGO and consolidation during the course of the disease, with the potential to improve the accuracy of clinical predictions. A Comparison of the Convenience, Quality of Interaction, and Satisfaction of Virtual and In-Person Healthcare Consultations: A Nationwide Study

Background: There are few direct comparisons of service utilization and patient-reported outcomes in patients attending medical consultations in person or virtually. This was a prospective, cross-sectional study of adults engaging with a healthcare practitioner via virtual or in-person consultations. Methods: Participants were recruited in person by convenience sampling between November 2023 and January 2024 across Saudi Arabia, and data were gathered on (i) basic demographic and consultation information and (ii) convenience, quality of interaction, and satisfaction with their consultations. Results: Of 3196 individuals who completed the survey, 28.7% had attended their most recent healthcare interaction virtually and 71.3% had attended in person. Participants attending virtual consultations were more likely to live rurally (69.0% vs. 21.9% for in-person consultations; p < 0.001). Virtual appointments were more common for primary care and diabetes/endocrinology but not surgical specialties (p < 0.001), and private apps and hospitals more frequently provided virtual appointments. Conclusions: Overall, patients found virtual consultations to be significantly more convenient, prompt, private, and well communicated than in-person appointments, translating into extremely high satisfaction (97.4% overall vs. 84.0% for in-person consultations; p < 0.001). This study provides population-level data on the current prevalence of telehealth use in Saudi Arabia. Further prospective research demonstrating the clinical noninferiority of telemedicine could help promote further uptake in specialties such as surgery. Full article

Introduction: Technological advancements and the COVID-19 pandemic have highlighted the importance of digital tools for patient care and rehabilitation. This study explores user perspectives on telerehabilitation, comparing it to traditional methods and identifying criteria for determining its suitability for different patients and clinical conditions. Methods: This study was carried out during the period of May–September 2021. Questionnaires were administered to 48 users in rehabilitation for audiophonologopedic and neurodevelopmental disorders in three rehabilitation centres in central Italy. Results: The user responses predominantly emphasize the benefits of time saving (68.75%) and cost-efficiency (37.5%), specifically regarding time saving due to travel and expenses incurred to go to where the therapy is carried out. The disadvantages include increased distraction (60.42%) in following the instructions remotely and logistic problems (39.58%). Patients with hearing loss were subjected to a larger number of telerehabilitation sessions, positively rating this alternative method. Patients with speech and language delay and autism spectrum disorder (ASD) prefer traditional treatment. Discussion: This study reveals a favourable perception of telerehabilitation as a therapy approach to be regarded as a supplement or temporary option to the irreplaceable face-to-face one. More research, as well as a larger sample sizes, will be useful to increase the significance of the correlations reported in this study. Full article

The management of patients with life-threatening respiratory disease in the ICU and at home has become increasingly of interest over the past decades. Growing knowledge supports the use of NRS, aimed at improving patient comfort and improving quality of life. However, its role during palliative care is not well defined, and evidence of support remains limited. The aim of this narrative review is to examine the recent evidence relating to the use of non-invasive respiratory support at the end of life, in order to clarify who benefits and when. The literature research was conducted on PubMed, using MeSH words. A review of the relevant literature showed that non-invasive respiratory support techniques for patients with life-limiting respiratory disease vary (from high-flow oxygen therapy to conventional oxygen therapy, from CPAP to NPPV) and each has precise indications. To date, from the hospital to the home setting, the monitoring and application of these respiratory support techniques have varied widely. In conclusion, the choice of respiratory support in this category of patients should be based on the technique that will optimize the comfort of the patient and improve the quality of their life. On the other hand, regarding monitoring, both telemedicine and ultrasound diagnostics help to satisfy the patient’s wish to spend the last period of his life in the home environment, to avoid inappropriately aggressive diagnostic interventions, and to reduce the high costs of hospitalized procedures in this category of patients.
