After observing the deterioration of condenser vacuum, the bypass v/v closed immediately but condenser vacuum did not improve. Meanwhile condenser back pressure increased up to 0.3 kg/cm2 and unit tripped on High condenser back pressure. After referring the condenser back pressure and running vacuum pumps inlet v/vs pressure recorded trend, the following observations were made :-At 02:50 hrs Condenser Back Pr :0.0857/0.0948 Kg/cm2, Vacuum PP-1/2 Pr :0.0817/0.91 Kg/cm2,-At 04:14 hrs Condenser Back Pr: 0.1/0.109 Kg/cm2 ,Vacuum PP-1/2 Pr :0.158/0.141Kg/cm2,At 04:52 hrs Condenser Back Pr: 0.291/0.298 Kg/cm2, Vacuum PP-1/2 Pr :0.337/0.332 Kg/cm2,(Just before tripping).It is clearly evident that at high condenser vacuum or low back pressure below 0.0857Kg/cm2, vacuum pumps seal ring had become unstable it might be due to low NPSH of seal water recirculation pump at such high condenser vacuum, therefore air ingress started from vacuum During condenser vacuum deterioration process, TDBFP-B exhaust line by pass v/v was opened as a result condenser vacuum dropped sharply and unit tripped on low condenser vacuum. Analysis revealed that the furnace pressure reached the tripping value after a small ripple of Furnace Pressure High to the tune of 40 mmwc indicating sudden dislodging of clinkers from furnace wall. The slagging tendency of the furnace is collaborated further with high SH spray (Left & right both) totaling about 120 T/hr.Slagging tendency may be due to combustion of high ash content in coal with the requirement of higher primary air flow and comparatively low secondary air in the furnace for maintaining full load. VINDHYACHAL 8 14-May-14 1:10:00 Unit was synchronized at 2314 Hrs and running at 120 MW load with Milling system AB, CD in service after attending BTL from S panel. It tripped at 2336 Hrs on Low Forward power protection.It was observed that the set pressure for HPBP opening was in manual mode and its set point was at about 55 ksc. As soon as the main steam pressure reached more than 65 ksc,( 10 ksc more than set pressure) fast opening of HPBP valves occurred. The machine shifted from Load Controller to Speed Controller and load reduced to zero leading to tripping of unit on Low Forward power protection. Root cause: - Set pressure of HPBP valve opening kept inadvertently in manual mode with the set point at 55 ksc. OPN 79. At 11:19 Hrs GT#3 tripped on Reverse Power (Power to trip) protection / Electrical Fault.On detailed checking it was found that just prior to GT#3 tripping, there was load fluctuation with hunting of gas control valves CV#165 (pressure control valve) and CV#174 (flow control valve). Fluctuation increased and gas control valve closed. Interruption of gas fuel supply, resulted in initiation of reverse power protection thus tripping the unit.After tripping of GT#3, operation checking of PCV # 165 and FCV # 174 was carried out and observed to be sluggish. Suspecting problem in the control oil system, the following were checked: NAS value of control oil of GT#3 was checked and found to be 8 (on higher side). The control oil quality of GT # 3 was last checked on 26th APR 2014 with NAS Value normal (< 7). The PQI (particle quantifier index) of GT # 3 also was last checked (sample dated 22nd April) in NETRA and found to be normal (value 4, normal not > 10). As during functional checking of control valve operation, C & I observed that both control valves, PCV (CV#165) and FCV (CV#174) were sluggish in operation, they replaced the E/H Servo valves of both control valves. Subsequently, the functional operation of both control valves after replacement of servo valves were checked and found OK. Further Control Valves line filter of CV#165 and supply filter of GT#3 control oil system were replaced with new ones. Thereafter control oil system was put in polishing. 10.0 Root cause : Choking of line filters of Control Valve CV#165 and Servo valve filter (subsequent to oil top up) caused restricted oil flow in Electro Hydraulic gas control valve. This caused improper operation of control valves & load fluctuation. Further closing of control valve caused Reverse power protection to operate. TG 5. AURAIYA ST-2 1-May-14 3:41:00 ST#2 was in service with GT#3 only as GT#4 was already under reserve shutdown. Upon tripping of GT#3 on REVERSE POWER (POWER TO TRIP)/ Electrical fault protection at 11:19 hrs of 01/05/2014, ST#2 tripped on GT No Load ST Trip protection as per logic. After normalization, ST#2 rolling started and synchronized at 15.16 hrs of 01.05.2014 with GT#4. Root cause: ST#2 tripped on GT No Load ST Trip due to tripping of GT#3 (GT#4 already under shutdown). TG 6. AURAIYA GT-4 4-May-14 209:15:00 LOW SCHEDULE GR 7. AURAIYA GT-3 12-May-14 2:15:00 ST#2 was in service with GT#3 only as GT#4 was already under reserve shutdown. GT#3 and ST#2 tripped on generator transformer HV neutral E/F protection.Heavy spark was observed on the 220 KV Auraiya Agra 1 circuit near Auraiya switchyard. Just before the incident, heavy storm started in the area. The sequence of events at AuGPS are as below.At 17.22.04.33 hrs. Agra 1 Main 1 & Main 2, zone-1 protection on B-Ph operated opening the B-Phase of the CB. At 17.22.04.99 B-Ph auto reclosed. At 17.22.05.69 Agra 1, Main 1 and Main 2 protection operated on R phase earth fault. R and Y Ph of the CB opened but B- Ph did not open.At 17.22.13.84 B phase earth fault appeared once again.At 17 .22.14.24 GT #3 tripped on Transformer HV neutral E/F.At 17 .22.14.24 ST #2 tripped on Transformer HV neutral E/F.Auto transformer 1 & 2 tripped on neutral E/F protection. During the incident it is noted that fault was changing from B Ph to R Ph. Initially when single phase fault appeared on the B phase of the Agra line 1, B- Phase pole opened and closed successfully on auto reclose. Again R phase earth fault appeared within the reclaimed time ( 2 seconds ) resulting into the initiation of three phase trip command. Only R & Y phase pole got opened & B phase pole did not open. For approx. 8 second, 0.135 Amp (CT Sec.) current was recorded in B Phase. Thereafter earth fault again appeared on B phase with approx . 8 Amp (CT Sec). GT #3 tripped on Transformer HV neutral E/F. ST #2 tripped on Transformer HV neutral E/F. While checking the Agra line 1 circuit breaker, trip coil of B Phase was found burnt and body was found cracked. (Both the trip coil i.e. TC 1 & 2 is mounted on the same moulding.Maintenance activities carried out earlier on the 220 KV Auraiya Agra 1 circuit at Auraiya are as: 1. OH of breaker mechanism with OEM : 02.06.2012 2. Annual maintenance and timing test of breaker : 22.06.2013 3. Annual protection testing : 22.06.2013 4. LBB test : 07.11.2013 Following investigations were carried out to ensure the healthiness of the system prior to restoration. 1. Testing of LBB relay : Found normal. 2. Scheme checking of LBB relay Operation as per scheme checked and found OK. 3. Inspection of control valve of B- Phase of breaker Found healthy. 4. Replacement of trip coil assembly of B phase. 5. Timing test of circuit breaker Found normal. Sustained fault in 220 KV Auraiya Agra circuit#1. During clearance of fault, GT # 3, ST # 2 and Autotransformers tripped at Auraiya end.
