Organ Donor


You may express your wish to donate your organs and tissues for transplant by designating the words "Organ Donor" to appear on your New Jersey driver's license or non-driver identification (ID) card. This designation is for individuals 14 years of age or older, whether or not the minor is emancipated.

Register for organ donor designation

Organ donor registration is available online or in-person at a motor vehicle agency. When registering at an agency, you must present the required 6 Points of ID well as verification of your address. Remember that you must confirm your decision each time you renew your driver's license or non-driver ID.

 

Effective October 1, 2017, if you wish to solely add the organ donor designation to your existing driver license or non-driver identification card, you may do so at no fee, upon surrendering your current driver license or non-driver identification card. 
You must present 6 points of ID and pay the required fees if in addition to the organ donor designation you need to:

  • Obtain an initial driver license or non-driver identification card.
  • Obtain a duplicate driver license or non-driver identification card.
  • Make any additional changes, such as an address change or upgrade.


Change an organ donor designation

To change your organ donor status on your driver's license or non-driver ID card, visit a motor vehicle agency. You must present the required 6 Points of ID as well as verification of your address. To submit your change request by mail, download a Change of Status form, complete and mail to:
