About licensure and certification
Federal regulation § 668.43(a)(5)(v) requires us to disclose info regarding our program and licensure and certification requirements. Our statement to meet this regulation is provided by the University of California at Licensure and Certification Disclosures.
Nondiscrimination policy statement
The University of California, in accordance with applicable Federal and State law and University policy, does not discriminate on the basis of race, color, national origin, religion, sex, gender identity, pregnancy1, physical or mental disability, medical condition (cancer related or genetic characteristics), ancestry, marital status, age, sexual orientation, citizenship, or service in the uniformed services2. Undocumented students

Students with disabilities
Services for students with disabilities at UCSF are coordinated by Student Disability Services (SDS). Students must self-identify and register with SDS in order to access accommodations at the School of Pharmacy. Accommodation plans and services are individually designed to match the disability-related needs of each student for their course or program with a goal to provide equal access to all aspects of the University environment. Accommodations are determined with consideration of documented need as described in disability documentation and student self-report, historical accommodations and services received, and the technical and academic requirements of the course or program. Typical accommodations might include (but are not limited to) test-taking arrangements, note taking services, alternate format media, ASL interpreters or real-time captioning, accommodations for the clinical environment, and accessible parking. Information Practices Act of 1977
The State of California Information Practices Act of 1977 requires the University to provide the following information to applicants for admission who are asked to supply information about themselves. The principal purpose for requesting information is to process your application for admission. Maintenance of this information is authorized by University policy. Furnishing certain information requested on this application is specifically noted as optional or voluntary. There is no penalty if you do not complete these items. Furnishing information that is not designated voluntary or optional is mandatory. Failure to provide such information will delay or may even prevent completion of the admissions process. Information furnished on this application may be used by various University departments for admissions and other student-related purposes, such as housing and financial aid. This information will be transmitted to the state and federal governments if required by law.
