Move 'Add Media' button into the TinyMCE toolbar

I have always been irritated by the placement of the Add Media buttons I think they ought to be in the rich editor toolbar (like they are when the editor is maximised, though the maximised ones currently don't work).
Now that there is only one button the placement seems even odder and more counter-intuitive, and I definitely think it should be moved into the toolbar.
This would also eliminate the interface change when the editor is maximised.

What does anyone else think about this? I'd change it myself, only I'm unfortunately not familiar with TinyMCE.

That would hide the button when using the HTML editor.

Well couldn't it be added to the HTML quicktags toolbar as well? Or is that just too complex?
