Are you trying to find information about your contact with the service?

  1. We can provide you with a letter to confirm the date and time of your contact with the service. To request this information, you will need to follow the steps below. Please note we are not able to provide any additional information about your contact with the service in the letter.

  1. We can provide case notes, transcripts, or copies of records when a subpoena or search warrant is served to us. Only your lawyer or police can organise a subpoena or search warrant. They can contact us by using this form and selecting ‘Subpoena or Search Warrant submissions’, where supporting legal documentation can be uploaded.

To protect the privacy and confidentiality of people contacting the service we require detailed information and relevant supporting documents as part of your application.

You will also need a safe email address to receive correspondence.

Follow these steps to request a letter to confirm date/s and time/s:

Before you submit your application:

  1. You will need to gather documents to prove your identity.
  2. These documents must meet 100 points of identification.
  3. At least one of these documents must have a photo of you – for example, a licence or passport.
  4. You will need to have copies of these documents certified. This means taking the original documents and copies of the documents to a Justice of the Peace or other authorised person to be certified.
  5. The photo identification needs special wording in the certification to prove it is your likeness.  Please ask the authorised person to write this statement on the copy: “I certify that this is a true copy of the original and the photograph is a true likeness of the person presenting the document as sighted by me”.

Submitting your application:

  1. Complete the request for information online form here. You will need to upload your certified documents to the online form.
  2. When we receive your documents, we will progress your request within five working days. We will contact you using the email address you provided. If there are any issues with your request, we will contact you using the same email address.

What is 100 points of identification?

  • You will need ONE primary document that has a photograph of you and other primary or secondary documents, so it adds up to 100 points.

  • We require this information to help protect and confirm your identity and to prevent anyone else from accessing your information.

What if I contacted 1800RESPECT and did not provide my personal information?

If you contacted the service and did not provide your name, phone number, pass phrase or any other information, we may not be able to help you. Without this information, we may not be able to match you to your records.

Can I amend my record of contact with the service?

  • We may be able to update your record with details such as a change of legal name, change of address, contact number or add details like your date of birth. We cannot amend your date or time of contact with the service.
  • Complete the request for information online form here, and select ‘Requesting to amend file’. 100 points of identification will be required to make any amendments.
