Kidney and cardiovascular specialists conducted a pilot study in 19 patients to show that the higher doses of spironolactone, which prevented reabsorption of excess salt in the kidneys and maintained potassium levels, could be used safely in these very-ill patients, who were admitted to the hospital with heart failure exacerbation and don't respond to conventional diuretics.

The renal tubules have specialised features that allow the reabsorption and secretion of solutes, and osmosis of water, as the urinary filtrate travels from the glomerulus to the collecting ducts and then out of the kidney.

In one study, it was found that those osteoporotic patients who took thiazide diuretic had 0.87 times the risk of having fractures compared to those who did not take the drug.2 Proposed mechanism through which Thiazide diuretic decreases the risk of having fracture is by not only increasing calcium reabsorption by the kidney but also by inhibiting sodium-chloride cotransport in osteoblast thus increasing calcium levels in the bones.3

To well understand the mechanisms on sweat secretion and reabsorption, the differential innervation of secretory coils and ducts in human eccrine sweat glands was investigated in the study.
