CALCULATION OF

INDIVIDUAL DOSAGES
and
IV FLOW RATES
• Formula: D x Q wherein D = Desired dose/Physician’s
order
S S = Stock on hand/Medicine on hand
Q = Quantity
• E.g.
1. A Vial of ampicillin contains 500 mg. of the medication
per 100 ml of solution. If a pt. requires a dosage of 2.5
g of ampicillin, how many ml of solution should be
administered?

2
• a. Convert 2.5 g to mg: 1 g= 1000 mg
1000 mg x 2.5 g = 2500 mg
1g
• b. Use the formula: 2500 mg x 100 ml = 500 ml
500 mg

3
• 2. Give Ciprofloxacin 750 mg BID P.O.
Stock on hand is 500 mg/tab. How
many tablets will your patient consume
in 1 wk?

4
• 750 mg x 1 tab = 1.5 tab x 2 (BID) = 3

500 mg
tabs/day

• 3 tabs x 7days (1 wk) = 21 tabs/wk

5
3. The infant weighs 8.4kg. The doctor
ordered Ampicillin 100mg/kg/day in 3
divided doses. Stock on hand is Ampicillin
500mg diluted in 5mL of sterile water for
injection.
•What is the total dose per day?
•How much are you going to administer
each dose?
6
• 100mg x 8.4kg x 1 day = 840 mg

• 840/ 3 = 280mg / dose

• 280mg/ 500mg x 5mL = 2.8mL

IVF CALCULATION

8
• Formula:
• gtts/min = TV(ml) x DF(gtts/ml)
Hrs. to run x 60 min/hr

• cc/hr = TV .
Hrs to run

• cc/min = cc/hr .
60 min/hr

hrs to run= TV
cc/hr
• wherein TV = Total volume
DF = Drop factor ( usually given ) 9
E.g.
• 1. A patient is to receive 2400 ml PNSS daily. If the IV
set delivers 15 gtts/ml:
• a. How many gtts/min. should the pt. receive?
• b. cc/hr
• c. cc/min
• d. hrs to run

10
• a.) 2400 ml x 15 gtts/ml = 36000 = 25 gtts/min
24 hrs x 60 min/hr 1440
• b.) 2400 cc = 100 cc/hr
24 hrs
• c.) 100 cc = 1.67 or 2 cc/min
60 min
d.) 2400cc = 24 hrs
100cc/hr

11
• 2. A 40 yr. old woman is to receive 1800 ml of IV fluid
over 12 hrs. via infusion pump. If the IV set delivers
10 gtts/ml:
• a. How many gtts/min. should the pt. receive?
• b. cc/hr
• c. cc/min

12
• a.) 1800 ml x 10 gtts/ml = 18000 = 25
gtts/min
12 hrs x 60 min/hr 720
• b. ) 1800 cc = 150 cc/hr
12 hrs
• c.) 150 cc = 2.5 cc/min
60 min

13
• 5. D5LR 960ml at 160 cc/hr
drip factor = 15gtts/ml
Compute for:
a. gtts/min
b. hrs to consume
c. due time if started at 10pm

14
• Compute for:
a. gtts/min
b. hrs to consume
c. due time if started at 10pm
• 160ml x 15 gtts x 1hr = 40 gtts/min
hr ml 60min
• 960 ml x hr = 6 hrs
160cc
• DT: 4am

PRACTICE

16
• 5. ORDER: Morphine sulfate 10STAT
AVAILABLE: Morphine sulfate 15mg/ml
a. How many ml should you give?

d/a (d/s)
10mg/15mg (1mL)
0.67mL

Check for the RR. Can cause respiratory depression.

17
• 8. ORDER: Aspirin 650 mg po stat
SUPPLY: Aspirin 325 mg/ tab

650/325 = 2 tablets

18
• 9. ORDER: Procardia (nifedipine) 20 mg po bid
SUPPLY: Nifedipine 10 mg/cap

• 20/10=2 tab per dose

• How many tabs per day? 4 tablets

19
• 11. ORDER: Potassium chloride 30 mEq PO OD
SUPPLY: Potassium chloride 20 mEq/15 ml

30/20*15= 22.5mL infuse PNSSiL
