1,000 micrograms (mcg) =1 milligram (mg) 1 milliliter (mL) = 1 cubic centimeter (cc)

1,000 grams (G) = 1 kilogram (kg) 1 teaspoon (tsp) = 5 milliliters (mL)

1,000 milligrams (mg) = 1 gram (G) 1,000 milliliters (mL) = 1 liter (L)

2.2 pounds (lbs) = 1 Kilogram (kg) 3 teaspoons (tsp) = 1 tablespoon (Tbsp) (15ml)

30 milliliters (mL) = 1 ounce (oz) 1 tablespoon (Tbsp) = 15 milliliters (mL)

2 tablespoons (Tbsp) = 1 ounce (oz)

H = dose on hand or dose on the label of bottle, vial, ampule.
V = vehicle or the form in which the drug comes (i.e., tablet or liquid).

RATIO & FRACTIONAL EQUATION METHOD

Formula = H:V= D: X Or = H/V = D/X

Where in:
D = Desired dose or dose ordered by the primary care provider.
H = dose on hand or dose on the label of bottle, vial, ampule.
V = vehicle or the form in which the drug comes (i.e., tablet or liquid).
Physician Order: Acetaminophen 500 mg, On hand/ Supplied : Acetaminophen 250 mg in
5 mL,
Calculate ml/dose ?

Solution:
Desired (D) = 500 mg, On hand (H) = 250 mg, Vehicle (V) = 5 mL

= 𝑎𝑚𝑜𝑢𝑛𝑡 𝑡𝑜 𝑎𝑑𝑚𝑖𝑛𝑖𝑠𝑡𝑒𝑟

= 10ml = Ans: 10ml/dose

Physician Order: Losartan 50 mg BID, On hand/ Supplied : Losartan 25 mg, Calculate


Tab/dose & Tab/day?

Solution:
Desired (D) = 50 mg, On hand (H) = 25 mg, Qty=1

= 𝑎𝑚𝑜𝑢𝑛𝑡 𝑡𝑜 𝑎𝑑𝑚𝑖𝑛𝑖𝑠𝑡𝑒𝑟

=2 = Ans = 2 Tab/dose & = 4 = Ans = 4 tab/day


Order: Erythromycin 750 mg On hand: Erythromycin 250 mg capsules
Calculate: Cap/Dose
Desired (D) = 750 mg , On hand (H) = 250 mg, Vehicle (V) = 1 capsule
Formula= H:V= D:X = 250mg: 1 = 750mg: X (dose to administer)
X= 750mg: 250mg = 3 Cap Ans: 3 cap/dose

Order: Digoxin 0.25mg Tablets On hand: 0.125mg


Calculate: Tab/Dose
Desired (D) = 0.25 mg , On hand (H) = 0.125 mg, Vehicle (V) = 1 Tab
Formula= H/V= D/X = 0.125mg/1 = 0.25mg/ X (dose to administer)
X= 0.25mg* 1 Tabs/ 0.125mg = 2 Tab Ans: 2 Tab/dose
Order: Drug A 3.6g IV daily Supplied: 1200mg/2ml Calculate: ml/dose ?
Desired: Have * Quantity = X (dose to administer)

Order: Drug B 750mcg PO Daily Supplied: Drug B 0.25mg/Tab

Q5. Order: 50units/kg/dose sublingual daily On hand/supplied: 5000 units/ml Weight of


the patients = 180 pounds Calculate : ml/dose

Q6. Order: 400mcg/kg/hr On hand/supplied: 250mg/ml Weight of the patients = 195 pounds
Calculate : ml/hr.
