 It is an official record of
relevant information to the
cases investigated which
serves as a vehicle to convey
to his superior.
 No matter how good the

investigation was conducted,


the effectiveness of the
investigator will be judged in
a large measure by his
•Purposes of IR:
 Record – provides permanent official
record.

 Leads – provides information to other


investigators.

 Prosecutive Action – provides the proper


authority statement of facts to base their
criminal, corrective or disciplinary action.
•Requirements of a good
IR:
 Accuracy – should be a true representation
of facts verified by witnesses and reliable
resources which includes both favorable and
unfavorable information to the subject.
 Completeness – should answer 5W’s and H
questions, establish the elements of the
crime and provide facts to prove it.
 Brevity – irrelevant and unnecessary
materials should be eliminated.
 Fairness – should take facts as he finds
them and if ever he has theories, it must
consistent with facts.
 Form and style - The arrangement of the
materials presented should be in a
manner which will make the report easy to
read. Forms include:
 proper paragraphing
 proper underscoring
 proper capitalization
 proper heading.

The report should be written clearly and


concisely in simple, direct standard English
and should be written in third person.
Investigator referring to himself as
‘This investigator’
•Criteria

 It must be grammatically correct


 Abbreviation must be used
appropriately and correctly.
 The report should avoid slang,
colloquialism or unnecessary
technical terms: example
‘Yagit’ , Salvage.

MEMORANDUM FORM

I – AUTHORITY:

 This section should contain a brief statement of


WHEN, WHERE and by WHOM the
investigation was made and should cite the
authority for making it. If the investigation was
made on the basis of the orders, this facts should
be stated, meaning the individual issuing the order
and date thereof. If it were made pursuant to the
written orders, specific reference to the document,
stating the date and the original directing
authority, whether local or higher commander, is
necessary.
 II - MATTERS INVESTIGATED:

This section of the report represents the


mission of the investigation investigating
officer. In essence, it answer the QUESTION:
“What is the investigation about?” Most of this
are written complaints or reports alleging that
some persons have committed some crimes or
impropriety, or the writer has been unjustified
treated. If the allegations are too long to cite, a
synopsis maybe set forth an appropriate
references made to the copy of papers in the
appendix upon which the investigation was
based.
 III- FACTS OF THE CASE:
 presents the real truth respecting the matter
investigated. The description
“COMPLETENESS & CLARITY” are
particularly applicable to this section.
 presents all the pertinent facts regarding
several matters at issue presented in the “
MATTERS INVESTIGATED” section.
 allegations should be properly presented to
tell the story and informs the reader which
allegation is considered if there are several
allegations.
 short cuts for the sake of brevity are never
permissible if they involved the omission of
the single fact necessary to establish the
truth and to convey that conviction to a
careful reader.
•IV – DISCUSSION:

 presents the appropriate presumptions and


inferences from all circumstances in the case to give
directing authority the clearest possible picture.
 should contain such related factual matters of the
argument in addition to the facts presented to
establish the conclusions and recommendations
based thereon.
 presents the mitigating and aggravating
circumstances.
 indicates if the case does not require special
discussion.
 discusses the character and records of the parties
involved.
•V – CONCLUSIONS
 a concise summary of the result of the
investigation directly consequent and
supported by facts contained in the
preceding parts of the report.
 represent the investigating officer’s
honest, well-studied opinion as to the
truth of each of the matters at issue.
 should contain NO apology, or
qualification such as, ‘IT APPEARS’, ‘IT
IS BELIEVED’ or ‘IT SEEMS
PROBABLE’.
 should not be repetition of the facts,
merely words differently.
•VI – RECOMMENDATIONS:
 constitute the investigating officer’s
judgment, in view of the facts he has
established.
 should contain the practical suggestion as to
appropriate actions to be taken to make
suitable disposition of all phases of the case.
 should follow, as far as practicable; the same
sequence as was used in the presentation of
the conclusion.
 should be consistent with and appropriate to
the conclusions.
NOTE: Paragraph subdivision should be
numbered consequently all throughout rather
than separately under each subdivision.
