OCCUPATIONAL SAFETY AND HEALTH
-RISK ASSESSMENT REPORT OF
2011E.C AND PROPOSED ACTION
PLAN

PREPARED BY: DR. ABEL A. (IF &EDMC)

OUTLINE
• MEASURMENT FOR RISK ASSESSMENT
• RISK ASSESSMENT RESULT AND ACTION PLAN FOR
2012E.C
MEASURMENT FOR RISK ASSESSMENT

1. Reporting Keys: Risk Assessment Evaluation Scale

The following keys were used for reporting results of the assessment of occupational safety and
health risk in the year 2011E.C:

Table 1: Risk Assessment Key (Scale)

O 1 2 3 4 5
Not Insignificant Minor Moderate Major Severe
applicable
MEASURMENT FOR RISK ASSESSMENT
Not applicable (0) - the process likely to present risk not undertaken in the facility

Insignificant (1) - No risk or the risk is low completely mitigated

Minor (2) - Risks exist in low quantities: exposures possible but unlikely in large quantities.
However, processes may present some risks whose results could be felt as minor on exposure

Moderate (3) - Significant risk exists; action plans must be developed and reviewed frequently

Major (4) – Non-compliance, risk Serious enough to warrant urgent changes in day to day
operations and exposure could be catastrophic: any negligence would move to catastrophic stage

Severe (5) - Catastrophic: Risk is serious enough to influence the hospital’s ability to meet
commitments; immediate intervention is required
