PERNICIOUS ANEMIA

PERNICIOUS ANEMIA
a decrease in red blood cells that occurs when the body cannot properly absorb vitamin B12 from the gastrointestinal tract. Vitamin B12 is necessary for the proper development of red blood cells.

VITAMIN B12
also called cobalamin, is a water soluble vitamin with a key role in the normal functioning of the brain and nervous system, and for the formation of blood. It is one of the eight B vitamins. It is normally involved in the metabolism of every cell of the human body.

INTRINSIC FACTOR
also known as gastric intrinsic factor (GIF) is a glycoprotein produced by the parietal cells of the stomach. It is necessary for the absorption of vitamin B12 later on in the terminal ileum.

MEGALOBLASTIC ANEMIA
megaloblastic anemia is a blood disorder in which there is anemia with larger-thannormal red blood cells.

PATHOPHYSIOLOGY
Predisposing Factors Age Race Family history Autoimmune disease Precipitating Factors Diet Surgery (removal of parts of stomach or small intestines) Drugs that affect the absorption of vit. B12

Lack of intrinsic factor in the stomach

Hypoxia Pallor Pail nail bed Loss of appetite

Decrease in vit B12 absorption Decrease in production of red blood cell Abnormal red blood cell

Disorientation Irritability and personality changes

Dizziness Fatigue Headache Pale skin or pallor Poor concentration Shortness of breath Weakness

Pernicious Anemia

Dementia Depression Loss of balance Memory loss Numbness or tingling in arms or legs

PERNICIOUS ANEMIA MANAGEMENT

Monthly vitamin B12 injections are prescribed to correct the vitamin B12 deficiency. This therapy treats the anemia and may correct the neurological complications if taken early enough. In people with a severe deficiency, the injections are given more often at first. Some doctors recommend that elderly patients with gastric atrophy take vitamin B12 supplements by mouth in addition to monthly injections. There is also a preparation of vitamin B12 that may be given through the nose. For some people, taking vitamin B12 tablets by mouth in a very high dose can be an effective treatment. A well-balanced diet is essential to provide other elements for healthy blood cell development, such as folic acid, iron, and vitamin C.

TEST TO IDENTIFY PERNICIOUS ANEMIA

Bone marrow examination (only needed if diagnosis is unclear) Complete blood count (CBC) Schilling test Serum vitamin B12 level

NORINE BARRIOS

TRACY PEARL TEMPERATURA

BSN 3-D
