MODULE: QUANTITATIVE RISK

ANALYSIS - Assignment
Loss of Containment Event

Objectives

The objectives of this case study are to:

Determine the frequency or probability of an on-site


explosion, resulting from; a major flammable product
release, and;

To identify and examine the consequences of such a


release, whilst also commenting on risk acceptability,
and mitigation.
Operating System
The operating system shown on the following slide
consists of the following:

1.A storage tank;


2.A system control booth;
3.An in line transfer pump;
4.A pump activation button;
5.A fuel pipeline system;
6.De-canting station
7.A manually operated shut off valve;
8.Power supply lines, and;
9.A level measuring system, which consists of the following; a float
mechanism, a level transmitter relay and an indicator system situated
within the control room (simple bulb type).

Site Brief

The facility is located near to; a small to medium sized community, the
residential boundaries of which commence at a distance of 150 meters
from the site, and lay in a “north easterly” direction.

The operation is used as a buffer facility for the receipt and subsequent
delivery of a highly flammable product. At one location adjacent to the
tank there is a manual de-canting station.

Surrounding the facility is a security fence, and fixed to the westerly


facing side of the fence is a sprinkler system.

Questions
1. Examine the potential failure modes of the operating system; using
Failure Modes and Effects Analysis (FMEA)

2. Construct a fault tree, starting with the top event of; an on-site
“explosion”

3. Use reliability data to calculate the expected frequency or


probability of such an event.

4. Using “Event Tree Analysis” (ETA) identify the “Pivotal Events” and
“Consequences” of a large flammable product release, in relation
to the community.

5. Comment on the acceptability and level of risk, and recommend


design or procedural changes to reduce that level.
