Last Minute Risk Assessment (LMRA)

• A LMRA is a final check for extra risks before, and during the work
task at hand.
• It means quickly reviewing the task and environment, mentally and
visually, to ensure all hazards have been identified, and control
measures put in place. This should also be done throughout the
duration of the work at each job step.
• This could include:
- Weather changes
- Increase in traffic volume
- Review of all necessary PPE
- Work method (change to job steps)

Emergency Response Plan

EVACUTION PROCEDURES

 Ensure all personnel at site is safely moved to designated assembly point.


 Before commence work, all personnel at site must be briefed the location of
assembly point based on site layout.
 Appointed first aider and fire fighters are responsible to take action on instruction
given by leader.
 Make sure one mobile phone is standby in vehicle for communication and
reporting purposes.
 Perform emergency alarm using hailer to attract all personnel attention at site.

EMERGENCY PROCEDURE

 Appointed emergency response personnel will be responsible as stated in


Emergency Plan.
 After assessing the situation, team leader will determine necessity of having
complete evacuation to avoid more injuries or fatality.
 All emergency contacts required will be displayed on information board or in safety
file.
 Emergency report such as fires, excavation collapses, falls or electrical
emergencies must be taken seriously and sound the hailer alarm immediately to
ensure all personnel at site are well notified.
 Appointed fire watchers will immediately try to put out the fire and the first aiders
will assist the victims.
 Ensure the Fire Brigade is contacted immediately and notify the location, the
number of victims injured or trapped and the type and the scale of the fire.
 All evacuated personnel need to stay at assembly point until the emergency case
is resolved and instruction from Shell management is given.

STRICT PROCEDURES TO FOLLOW ONCE THE ALARM GOES OFF

 DO NOT PANIC.
 STOP ALL WORK IMMEDIATELY.
 DO NOT DELAY THE EVACUATION
 DO NOT RE – ENTER THE SITE UNLESS THE AREA HAS BEEN CONFIRMED
SAFE.
 HELP THE INJURED GET TO SAFETY AND ASSIST THE PEOPLE WHO DON’T
KNOW WHERE THE ASSEMBLY POINT IS.
 DO NOT LOCK BARRICADING GATE TO ALLOW EMERGENCY SERVICES TO
ATTEND TO THE SITUATION QUICKLY.
 TRY TO WRITE DOWN THE EVENTS THAT LEAD TO THE INCIDENT
OCCURING AND ALSO MAKE NOTE OF THE SITUATION.
 WAIT FOR FURTHER INSTRUCTIONS FROM MANAGEMENT OR THE
PROJECT MANAGER.

PERSONNEL HEAD COUNT


 All personnel must be accounted for thoroughly to make sure that no one was left
behind.
 Site retailer or management must also do a head count of all their staff.
 If possible check work area, toilets and the shop area to make sure that there is
nobody inside.
 If there are missing persons, the emergency services should be notified that a
search team will be required.

ROUTINE DUTIES
 All staff on the work site will wear helmets, safety shoes in an event requiring
emergency rescue.
 A copy of the rescue plan must be available on a clip board as well as a check list of
all personnel on the work team.
 Perform routine drills to make sure that people are aware and will be prepared for in
case there is a real emergency.
 Make sure that there is a safe exit from the assembly point allocated.
 Make sure that first aiders and fire fighters are prepared at all times.

• STEP 1: Pre- start work


- Fully barricade/ enclose work area
- Make sure to follow traffic plan management.
- All the signage to indicate work in progress must to be place.
- Erect scaffolding or mobile ladder, make a safe by check and green tagged (if required).
- Ensure the flag man will monitor surround from others.
- Inspect the lorry crane by referring to lifting form. (as attached)
- Make sure to follow all planning to place the cabin

• STEP 2: Loading and Unloading cabin


- Lorry crane driver must communicate with conductor. To ensure the sign to open and
low down all ball and rope in good way.
- The flagman must ensure surround safe all time.
- Add stone or block bar under the cabin to avoid damage to cabin and road surface.
- After tight the rope to cabin and loading. Ensure the rope tight in good.
- The same things shall applied during unloading.
