What is an
affidavit of
complaint?
• A sworn statement prepared by someone who wishes to
file a legal complaint.
• It becomes the basis for the case, providing basic
information about the facts of the matter and
outlining the nature of the case.
The following are the contents of the affidavit
of complaint:
• Begin with a title: “Affidavit of Complaint”
• Include a statement of identity: “I, [your name],
swear under penalty of perjury that the
following statements are true and correct to the
best of my knowledge and belief.”
• Provide your personal information, including
your name, address, and contact information.
• Identify the respondent or defendant, including
their name and address if known.
The following are the contents of the affidavit
of complaint:
• Describe the incident or issue that is the subject of
the complaint in detail, including dates, times,
locations, and any other relevant information.
• State the legal basis for your complaint, such as a
violation of a specific law or regulation.
• Include any supporting documentation or evidence
that you have to support your complaint.
• Sign and date the affidavit in front of a notary public
or other authorized official.
