Introduction….. Prescription is task of a doctor in which it is the process of giving treatment to the diseases. This process involves analyzing and writing patient history or patient chief compliance, writing diagnose, give appropriate treatment (Rx) to the diagnosed diseases, making some calculation of dosage specially for pediatrics, analyzing drug appropriateness (contraindication, indication, allergy, and extra), and finally transmitting the treatment to the dispenser. Introduction….. As study reveal a lots of deaths occur each year in the world due to medication errors. Even if we do not get any study in Ethiopia it can be deduced that many people’s die each year in our county. This problem makes us to look very carefully and to explore out how to solve these problems and how to improve it using technology and by applying computer applications concepts on current prescription process in order to change that manual process in to moderate electronic form. Introduction Current systems for prescribing and administration of medicines in Ambo referral hospital are based on a model established over 46 years ago. Since then medications used have grown in number and complexity, with a resulting potential for greater risk to patients. The current system has the following problem Service related problems Registration related problems Generating report Storage What is Electronic prescription ? E-prescription is the processing of a drug prescription in an electronic form, starting from the medical professional prescribing the drug and ending with the actual dispensing to the patient, including the process of drug selection and demand, with the data always stored electronically. E-Prescription is a tool for prescribers to electronically send an accurate, error-free and understandable prescription directly to a pharmacy from the point-of-care. Kilbridge and Dladysheva define e-Prescribing as “entering a prescription for a medication into an automated data entry system (handheld, PC, or other), and thereby generating a prescription electronically, instead of handwriting the prescription on paper” (Kilbridge & Dladysheva 2001). General Objective of the projects Project Team has the general objective of developing the new web based electronic prescription system for Ambo referral hospital that can address the problem with the current system mentioned above. Convert manual system prescription process and explore out to a system that is a tool for prescribers to electronically send an accurate, error-free and understandable prescription directly to a pharmacy from the point-of-care. Specific Objective of the projects… For the purpose of achieving the above objective the following specific objectives are identified • Reviewing the current system to the problem • Study each activity of the current system to identify the problem • Identify problems of the existing system • Suggest alternative solution to address the existing problem • creating a model Specific Objective of the projects • designing system architecture of the system • testing the operation of system • documenting the whole system • designing user friendly interface • designing back end database that can hold all the information • To generate report done on the system. Scope of the projects Our developed E-prescription system will have functionality for entering prescription for a medication in to an automated data entry system (especially pc, desktop and tablets). generating a prescription electronically, instead of handwriting the prescription on paper. Developing a tool which allows the patient to improve the safety through electronically checking patient allergies contraindication etc. security and accuracy of his prescriptions, save his time during handling prescription renewals by making it electronically with his pharmacy. Generally, The system is designed: To allow The prescriber to generate a complete active medication list incorporating electronic data received from pharmacy about medicine, from porter (nurse) about patient demography and if the patient is out patient about patient history. To allow the subscriber to Select medications, printing prescriptions, electronically transmitting prescriptions . To allow the subscriber to conducting all safety checks (safety checks include: automated prompts that offer information on the drug being prescribed, potential inappropriate dose or route of administration, drug- drug interactions, allergy concerns, or warnings or cautions). In addition to this the system designed to provide information related to the availability of lower cost, therapeutically appropriate alternatives (if any). significance of the project Our E-Prescribing system can deliver benefits for those who prescribe, administer, screen and dispense medication, as well as those who need to see medication records on a “read only” basis. During Prescribing • When prescribing, the system can help in a number of ways and various situations: • Providing prescribers with access to decision support . • Allowing access to electronic patient records (as well as medication records) . • Removing the need to rewrite medication charts, and the associated potential for transcription errors . • Supporting direct communication of prescribing information to pharmacy and other departments . • Allowing the easy and direct production of discharge prescriptions Review and dispensing of medication
Our system can have significant positive consequences for pharmacists
as they check (“screen”) medication orders, correct errors and follow up queries has the following benefit: Access to electronic patient records specifically patient demography information and Allowing easy review of previous medication history Many basic prescribing errors (missing information, transcription errors from one chart to another, prescribing doses that are not possible with the dosage forms available etc.) are likely to be prevented and will not need to be corrected Identification of each individual prescriber to allow follow up of queries Viewing medication records Our system can help by: • Allowing easy review of current and previous medication history • Access to electronic patient records specifically patient demographic information (as well as medication records) • Removing the problem of missing drug charts or not being able to access those that are in use by other healthcare professionals Communication between all prescription stakeholders Supporting the transfer of information’s from dispenser to prescriber, administrator and porter between leader and member. ?
