Note: The value of 150 in Fried’s rule was an estimate of the age
(12.5 years or more 150 months) of an individual who would
normally receive an adult dose, and the number 150 in Clark’s rule
was an estimate of the weight of an individual who likewise would
receive an adult dose.
 
SAMPLE

PROBLEMS:
1. If 150mg/kg body weight is the usual initial dose. How many
milligrams should be administered to a 154 lbs patient?

By using the table, calculate the IV drug dose for a 3-pound 3-ounce
neonate.
3 pounds = 3 x 454 g = 1362
3 ounces = 3 x 28.35 g = 85 g
Weight of neonate = 1362 g + 85 g = 1447 g
1447 g/1000 = 1.447 kg
30 mg/kg X 1.447 kg = 43.4 ng every 12 hours, answer.
SPECIAL DOSING CONSIDERATIONS IN CANCER
CHEMOTHERAPY
Chemotherapy
 Applies to the treatment of disease with chemical drugs or
chemotherapeutic agents
 Primarily associated with the treatment of cancer patients and is
considered the mainstay of such treatment in that it is effective in
widespread or metastatic cancer, whereas treatments such as surgery
and radiation therapy are limited to specific body sites.
 Chemotherapeutic agents most often are administered orally, by
intravenous injection, or by continuous intravenous infusion.
SAMPLE
   PROBLEM:

1. Regimen:
Cycle: 28 days; repeat for 2-8 cycles
Vinorelbine, , IV, D 1, 8, 15, 22
Cisplatin, , IV, D 1
 

For each of vinorelbine and cisplatin, calculate the total
intravenous dose per cycle for a patient measuring 5 feet 11 inches
in height and weighing 175 lbs. From the nomogram for
determining BSA: a. find the patient’s BSA and b. calculate the
quantity of each drug in the regimen.

a. BSA =

b. Vinorelbine: 25 mg x 2 (BSA) x 4 (days of treatment) = 200mg


Cisplatin: 100mg x 2 (BSA) x 1 = 200mg
