INTRODUCTION
Optimization in radiotherapy is related with determining the ‘best possible’ radiation dose distribution for a
given treatment situation.
the dose gradient across the tumor should be minimized,
tumor dose relative to the largest incident dose should be maximized,
integral dose should be minimized,
the shape of the treated area should be matched to that of the chosen treatment area,
the dose to particular vulnerable regions should be minimized
a higher than normal dose to regions of possible direct extension or lymphatic spread should be allowed

Selection of field direction and width
The great majority of radiation treatments have field direction and size determined at the time of treatment
simulation, and for the above reasons it was decided at present not to attempt selection of these parameters
in the development of a new technique.

Wedge selection
Quadratic programming is performed for each arrangement and that with the lowest variance is selected as the optimum
plan.

Algorithms
In quadratic programming the linear constraint equations together with the condition that the variables are
positive, define a region within which the minimum value of the objective function is to be found. If no
constraints are placed on the variables, the solution may have all variables equal to zero as this produces
the minimum value of the variance.
The planning technique
Tumour points lie within the tumour or target region of the patient, and the planning program
automatically selects a set of points arranged in a systematic manner throughout the region.
The beam weights have to be constrained to be non-zero and the method chosen include an equality
constraint to make the average dose to the tumour points equal to lOO%

Constraint points are not automatically defined. They are employed after the first execution of the
quadratic programming should the resulting plan show dose regions that clinically are unacceptably high.
After execution of the quadratic programming, the operator is informed of the standard deviation of the
dose to the selected tumour points.
The figure of merit used to assess the quality of a plan, and is used in preference to the variance as the
number of tumour points included in a calculation may be altered.
In wedge selection, the five best plans are determined and the operator has the choice of selecting any
one of these
