8 Recurring Process Safety Defects

1. Operating equipment before understanding the safety critical aspects of operation and before verifying
the safety critical 'lines of defense' are in place and functional.

2. Starting or operating equipment when safety critical instrumentation is bypassed, out of calibration or
out of service.

3. Allowing critical operating / maintenance positions to be filled by people who haven't demonstrated 1)
knowledge of their operating hazards, 2) skilful ability to quickly and effectively respond when those
hazards occur and 3) fitness for safety critical duty (physical, emotional and alert).

4. Tolerating or condoning violation of operating procedures and/or placing no particular emphasis on


adherence to all safety critical aspects of procedures.

5. Initiating high risk / high consequence 'unusual or abnormal operations' without first completing a
competent start-up procedure and review (incl. CoW).

6. Allowing people to open closed systems without first taking the steps needed to prevent injury / incident
/exposure if the contents are unexpectedly released.

7. Allowing flammable / hazardous vessel and tank filling activities without taking the steps necessary to
prevent overfill, prevent overpressure or planning response for possible overfill.

8. Operating systems that can allow flammable / hazardous materials to flow into auxiliary and non-
hazardous services.
