The community health nurse may also develop and implement programs that reach to large group of community. Major types of community health promotion program include school, work place , faith community, hospital, and community wide program. School-Based Health Promotion. Workplace Health Promotion. Health Promotion in Faith Communities. Hospital – based Health Promotion Program Health promotion in Geopolitical communities. Health Promotion And Nursing Practice Responsibilities Of The Community Health Nurse In Health Promote : 1. Assist the individuals and groups to assess their level of wellness , health status and health goals. 2. Advocate to assist with accessing health resources and act as a liaison between families and community system. 3. Provide health education including identification of option for health care , teaching parenting skill, facilitating communication and teaching milestones across the life span. 4. Assist the client to identify strength and coping skills. 5. Assist with establishment of goals and priority for change and life style modification. 6. Evaluate intervention and monitor goals. 7. Conduct research in health promotion to influence policy information. 8. Act as a role model for promotion and protection of health. Appraisal and Assessment Methods used to assess health promotion behaviors and health risk in community Health risk appraisal or health hazard appraisal : Collect and organize personal data to provide an accurate, individualized assessment of risk factors that lead to health promotion. And stimulate the necessary behavioral change that may reduce health risk. Stress risk assessment: ( assess stressful effect of significant life events on person ) Life time health monitor program: (establish lifetime of health program) Life style assessment and wellness inventories : (it focus in health promotion rather than identifying risk factors for specific disease . and focus on daily behavior that affect on health). Developing A health Promotion Plan of Care 1. Summarize and review of information collected in the assessment. 2. Identification and reinforcement of strengths. 3. Development of health goal and appropriate life style change option. 4. Identification of desired health and behavior outcomes. 5. Design of behavior change plan . 6. Creation of reward system. 7. Review environmental and interpersonal facilities and barrier to behavior change. 8. Development of an implementation time frame. 9. Commitment to the proposed behavior change goal and support . Intervention strategies 1. Information dissemination 2. Life style modification 3. Environmental restructure 4. Strength social support 5. Public – private partnerships for political action.
