Introduction

A majority of cancer patients need radiotherapy during the course of


treatment. In a developing country like India, teletherapy using cobalt-
60 is the most cost-effective and relevant method of treatment. As per
WHO a developing country should have at least one teletherapy
machine for a population of one million.
Despite of technological and practical advantages of linacs over
telecobalt-60 machines, the latter still occupy an important place in
radiotherapy in developing country, because of:
 Lower capital and installation cost.
 Lower servicing and maintenance cost.
 Lesser dependence on reliable electrical power.

Different Parts of a telecobalt machine
A. Head Assembly- It is made up of Tungsten or Depleted Uranium to
shield the source off position. A mechanism is responsible for
bringing the source in front of the collimator opening to produce
the gamma ray beam. Currently, two methods are used for moving
the source from the BEAM-OFF into BEAM-ON position and back:
1. Source on a sliding drawer.
2. Source on a rotating cylinder.
 When then source is in BEAM-OFF position, a light source appears in the
beam on position above the collimator opening, allowing the
visualization of optical field defined by the collimators and shielding
blocks.
 The head may be rotated manually ± 180° in either direction. The head
rotation angle can be read from display monitor as well as circular
scale.
 Collimator- A manually adjustable, divergent asymmetrical collimator
defines the X and Y axes relative to the collimator axis. The collimator
may be rotated ± 180° from its central position and locked in any given
position.
 Optical Distance Indicator- It projects a scale on the patient’s skin
surface.

B. Gantry- The gantry rotates in both clockwise and counter clockwise
and is capable of continuous rotation. The angle of gantry is displayed
on a circular scale located at the centre of the gantry. The gantry has
a counter weight in order to stabilize the rotation of the source head
about the gantry axis.
