Before entering a hospital to the inpatient


unit , a patient is given an admission form. It is
used to obtain information about case history, in
order to determine a patient goes to
appropriate ward.

He/she will be accepted by nurse in a


ward. Based on an observation chart, a case
history sheet and care plan report, the nurse
will develop a precise plan of care. Therefore a
nurse needs to do an interview.
ADMISSION UNIT
• The nurse will obtain the information from the
patient before entering inpatient unit
• At admission unit there might be a nurse there
• The information are about: patient
characteristics, illness history, family
illness history, habits, administration
process
• The patient will be informed: General condition,
plan of care, discharge planning, inpatient cost.
