 A failure mode and effect analysis (FMEA) is a
type of hazard review that probes the failures
of components within a process or system and
the resultant effects of those failures
 FMEA methodology is largely equipment
oriented
 This technique is useful in analyzing single
failure modes that result in unwanted
situations and not efficient for reviewing large
number of multiple component failures

◦ All the major equipment are listed in the order in
which they appear on the P & I Diagram

◦ Each piece of equipment has to be “checked off”


for its failure modes

◦ Ensure that the equipment is identified in ways


(V-141) that the reviewers can understand and
usable later by others who act upon
recommendations

 Determine the failure modes
o For each component or piece of equipment listed, determine
all of the possible failure modes that are consistent with the
equipment operation

 Determine the Effects of the Failures


◦ The review team shall carefully examine and consider
all the immediate effects of those determined failure
modes
 Judge the Risk
◦ The review team then makes a judgment about the risks
around each of the equipment failures and associated
effects and classify its severity/consequences.

 Recommendations for Action
◦ Based on the identification of failure modes
determined for each of the situations, the team
then shall make recommendations.

◦ Discussions and brain storming sessions at each


failure modes are carried and suitable preventive
techniques/suggestions will be recommended.

Reporting the Results

◦ The information developed during the


analysis needs to be communicated for
important findings and it shall be directed
towards its implementation to overcome
the failures.

◦ A tabular nature of worksheet is prepared


by the team and submitted for all of the
findings
