Main complaint : Stab wound at left forehead


History taking : The condition had been apparent for 1 hour before the
patient was taken to the hospital. There were no events
of unconsciousness and vomitting. Previous medical
care was carried out at Ibnu sina general hospital.
Mechanism of : The patient was riding a motorcycle when suddenly he
injury was punctured by an arrow from unknown direction.
Sustained Injury : Left Forehead
Symptom & sign : Pain
Examination : Physical examination, Laboratory examination, Thorax
done X-Ray, CT Scan.

C: BP :120/ 70 mmHg, HR :90 x/minute, regular, adequate

D: GCS 15 (E4M6V5), pupil equal 2,5 /2,5 mm, Light

I : : Seen punctured wound size 0,5 x 0,5 cm with
arrow remains, edema (+), hematoma (-), active
bleeding (-), deformity (-)

HGB : 15,9 g/dL

HCT : 45 %

Blood Sugar : 87 mg/dl
