Family Health Nursing Process
It is a systemic approach to help family to
develop and strengthen its capabilities to meet its health needs and solve health problems. There are five distinct phases of family health nursing process: 1) Assessment phase 2) Diagnosing phase 3) Planning phase 4) Implementation phase 5) Evaluation phase Assessment (1st phase) It is the systemic and continuous collection, organization, validation and documentation of data. Assessment phase (Steps) 1. Data collection General information Family structure and characteristics Family life and culture Socioeconomic aspects Health and medical history Environmental factors 2. Data collection methods and techniques Observation Questioning Conversation/discussion Listening Review of family health record Examination investigations Cont 3. Organization of data 4. Validation of data 5. Documentation of data Diagnosing (2nd phase) It is the second phase of nursing process, in which nurse interprets assessment data, identifies client strengths and health problems and formulates diagnostic statements. Diagnosing phase (Steps) 1. Analysis of data To classify the problem as actual or potential. 2. Identifying the clients strengths 3. Making diagnosis Family health diagnosis is the written statement of family health problems/health needs which are assessed form analysis of data collected. Examples Inability to make decision to take any action related to misconception about family planning. Inability to recognize the exact nature of problem related to lack of knowledge about the nature and extent of problem. Planning (3rd phase) Planning is a deliberative systematic phase of the nursing process that involves decision making and problem solving. Planning phase (Steps) 1. Analysis of diagnosed health problem 2. Establishing priorities 3. Setting goals and objectives 4. Formulating family health nursing care plan Implementation (4th phase) Implementation is the phase in which the nurse puts nursing care plan into action. It involves doing, delegating and recording. Implementation phase (steps) Review and revise Mobilization of resources Implementation Documentation. Evaluation (5th phase) Evaluation is the fifth and last phase of the nursing process, in which nurse determines the clients progress toward goals achievement and effectiveness of the nursing care plan. Evaluation phase It includes the comparison of nursing actions and outcomes with the standards and criteria. The reason of this step is to recognize that either the expected goals are achieved or not.
