(EHRs)
Refers to the systematized
collection of patient and
population electronically-stored
health information in a digital
format. These records can be
shared across different health
care settings. Records are shared
through network-connected,
enterprise-wide information
systems or other information
networks and exchanges.
The EHR includes all relevant

information pertaining to the health
of an individual including his/her
demographics, problems, prescribed
medications and treatment plan, vital
signs, past medical history,
immunizations, laboratory and X-ray
reports or any such information that
is required to be easily accessible by
any healthcare provider at any point
of time in the lifespan of the
individual.
A summary of the patient's

medical history, and
documentation of each event,
including symptoms, diagnosis,
treatment and outcome.
Relevant documents and
correspondence are also
included.

The Philippines government is

gradually mobilizing its rural health
units (RHU) to adopt an electronic
patient record system in their offices.
The UP Manila National Telehealth
Center has initiated and developed a
computerized patient record system
known as Community Health
Information Tracking System (CHITS)
aimed to provide Filipinos quality
health care and management even in
hard-to-reach areas.

How does CHITS work in a health

center?
As the patient comes in, his/her record is
retrieved in seconds. If the patient is new
he/she would be registered that would take few
minutes.
CHITS is accessible in all departments within
the health center so once the patient
information is processed in the admission area,
the doctor and the other health provider are
promptly informed of the patients chief
complaint ,vital sign and history without
having to wait on the patient.
Open Medical Records System (Open

MRS)
Another electronic health record

program introduced by The
National Telehealth Center is the
Open MRS.
It is a software platform and a
reference application which
enables design of a customized
medical records system with no
programming experience.
It involves minimal use of free text and

maximum use of codes and standard
information.
At its core is a concept dictionary which
stores all diagnoses, test, procedures,
drugs and other general questions. It is a
client- server application and works best
in a networked environment.
