DISCHARGING A CLIENT FROM A HEALTH CARE AGENCY

Purposes:

To return the person to a state of independent living. To provide continued care by home-health nurses or with family assistance.

Assessments:

Review the discharge planning form. Discuss the possible options after discharge. Determine if the patient or his family has the knowledge and skills necessary to maintain or improve the current level of health. Explore dietary changes, medication administration, use and care of technical equipment.

Identify if assistive devices such as walker or commode have been obtained. Check to see if the home environment has been modified to avoid structural barriers such as stairs. Assess if patient can obtain transportation for subsequent health care. Check if the patient has had contact with a social worker for assistance with finances, insurance or medicare. Determine if patient has been referred to appropriate community services. Interview the patient to determine his perceptions concerning discharge and recovery. Explore the patients and familys attitudes and beliefs that affect health and illness.

1.Check to see that the patient has a written discharge order. 2.Make sure the patient or support person has discharge instructions (i.e. regarding diet, medication, etc.) 3.Instruct the watcher to go to the accounting section for financial assessment and payment. 4.Assist the patient to dress and pack belongings. Make sure the patient has all personal belongings.

It is the physicians responsibility to discharge a patient. The patient or support person should be able to continue with necessary care after discharge when properly instructed. These actions help to avoid legal problems

Assisting the patient conserves patient strength. Time and trouble are saved when the patient leaves with all belongings. Makes the nurse aware of the time the patient was discharged. Makes the nurse aware of the time the client was discharged.

5.Present discharge slip to headnurse or nurse on duty for signature

6.Transport and assist the patient into the car as necessary and give the discharge slip to the security guard.

Assisting the patient conserves the patients strength. Such assistance done in courteous manner. Helps the patient feel that personnel are interested in his welfare.

7.Strip the linen and clean the patients unit. Wash your hands.

To prepare the unit for another patient.

8.Make necessary recording on the patients record and complete the discharge summary.

It is important legally that the patients permanent hospital record is complete.
