Assessment Subjective cues; Tumataas ang BP ko as verbalized by the patient. Objectives cues; - Experience pain, back of the head.

- Irritable - Restlessness - Weakness - VS; BP 140/80 mmHg - fatty foods

Nursing diagnosis Hypertension related to cardiac output.

Planning Short term: After 2-3 hours of nursing intervention the patient BP will be able to in the normal range (120/80 mmHg).

Intervention Identify factors, such as age, functional decline, clients resistive to efforts, painful conditioning, breathing problem etc. Implement physical therapy/ exercise program with the client and other team members. Review previous life situation and role charges with clients. Encourage the patient to limit eating fatty foods. -

Rationale That could block/ affects desired level of activity.

Evaluation Short term: After 2-3 hours of nursing intervention finally the patients BP will be able to in the normal range (120/80 mmHg).

Long term; After 1 month of nursing intervention the patients BP will be able to maintained within the normal range (120/80 mmHg)

Coordination of program enhances likelihood of success.
