Surgery Modifiers
Modifier – Modifier is a two-digit alphanumeric word
QS, QZ, QY, QK, P1-P5, AA Modifier 22 – Increased time, Complexity of the procedure Eg – If the dr is performing hepatectomy (Left lobe) and due to excess fat the dr is having some difficulty in performing the proc. Which increases the time of the surgery, hence in this case we will use 47125 – 22 Modifier 23 – Unusual procedure(In which anesthesia is not usually needed but due to unusual circumstance the general anesthesia is given) Modifier 47 – Anesthesia given by surgeon Modifier 50 – Bilateral Procedure (Procedure performed on both sides of the body ) RT – Right Side of the body LT – Left side of the body 20610 – RT 20610 – LT 20610 – 50 Modifier 51 – Multiple procedure (Procedure done on the body either multiple times at different location or different procedure at the same location) Modifier 52 – Reduce Service (Service only Partially performed) Partial Tooth extraction CPT D7140 – 52 Modifier 53 – Discontinue service The service is cancelled due to some reason Modifier 58 – Staged procedure (the surgery is performed in parts) Modifier 59 – Distinct procedure (When we want insurance to pay the service of the separate procedure which is generally bundle in the large procedure) Modifier 78 – Unplanned return of the pt in postoperative period to the operating room during the post operative period in related condition Modifier 79 - Unplanned return of the pt in postoperative period to the operating room during the postoperative period for unrelated condition Evaluation and Management(E/M) 24, 25, 57 Modifier 24 – Separate service is given in E/M within Global period of the surgery Modifier 25 – Separate service is done in E/M Modifier 57 – Scheduled for surgery (Decision for surgery) 99024 – Global period E/M
