suspension and am permitted by the rules of my Home Federation ,
to participate in tournaments in Singapore.

I have read and understood the content of the Singapore Hockey Federation’s Personal Data Privacy
Statement and Notice. By providing my personal data and by proceeding with my application, entry form,
registration form, event-signup or transaction or using the services of the Singapore Hockey Federation, I
hereby:

(a) Consent to the collection, use, process and disclosure by or on behalf of the SHF of my personal data to
