Please take the time to read first the Criteria before evaluating your members. Scoring will be 4 being the Highest and 1 being the lowest.
Adding all scores together, base the total on the points provided below in red font and supply it to the TOTAL SCORE in the Table below.

Scoring Guide and Interpretation

• 18-20 points (Outstanding Performance) – Exceeds expectations in participation and teamwork.


• 14-17 points (Good Performance) – Meets expectations with consistent effort and contribution.
• 10-13 points (Satisfactory Performance) – Shows some participation but needs improvement.
• 5-9 points (Needs Improvement) – Minimal effort and contribution; needs significant improvement.

the TOTAL SCORE and when done, submit it to your Class Leader.
Class Leader: Kindly list your group leaders (LAST NAME, FIRST NAME, MI) supply the points base on your evaluation, get
the TOTAL SCORE and when done, and submit it to your Class Secretary.
Class Secretary: Compile and submit a soft copy of the scores of the whole class to your teacher.

Class Leader will be graded by the Teacher for the performance and output presented by the class.
