PURPOSE OF TRAINESS RECORD BOOK

UNFORGETTABLE
This Trainees Record Book (TRB) provides an updated documentation of your
achievements of competencies/tasks as agreed within the training plan. It is purposely
designed to:
1. Record and monitor the student’s progress in the performance and completion of S
various tasks per learning outcome;
2. Serve as a personal multipurpose document reflecting essential information about
the trainee and the training undergone. Best lesson statement learned:

 Present to concerned parties the list of specific trainings completed by the student
 Assess skill levels should this be required for employment or further trainings

INSTRUCTIONS ON THE USE OF THE TRAINEES RECORD BOOK Most exciting event with my classmates:
This TRB serves as a record of your training accomplishments while undergoing
training in PSAT. It is therefore important that you and your instructor enter all necessary
data accurately and legibly in ink, avoiding any corrections, erasures, unnecessary marks
and perforations if possible.
First successful task completed satisfactorily:
The SRB contains all the required competencies and their respective tasks that
you should perform to gain the necessary knowledge and skills for the Computer
Systems Servicing (CSS) NC II qualification. All you have to do is enter the date
you actually started the task required. Upon completion of the task, present your
output or quality of performance to your Instructor for evaluation and then based on his Most difficult PC problem solved:
recommendation the actual date of completion should be entered, followed by the
Instructor’s Remarks and his signature for confirmation.
After achieving all the Learning Outcomes for each Core Competency, you should
fix your signature at the bottom of the page. Likewise, your Instructor will confirm your Core
Competency gained with his signature. Finally, the TIS will sign your TRB for the Most unforgettable Instructor’s remark:
school’s confirmation of your newly acquired competency.
Finally, after the course duration, your instructor will collect your TRB for
safekeeping as this will serve as a component of your permanent record on file.
