1- What is Software Quality Assurance (SQA)?

 Definition: SQA is a process-centric approach to ensure that software meets quality


standards and fulfills requirements. It encompasses planning, defining standards,

 Goal: Build confidence that the final product is reliable and meets user expectations,
not just through final testing but via preventative processes during development.

2- Quality Models Introduction

 Why Quality Models: Quality models provide a structured framework to define and
evaluate software quality attributes. Instead of subjective opinions, they offer standard
criteria for what “quality” means in software.

 Industry Standards: The ISO/IEC standards (like 9126 and 25010) are widely used
models that list the characteristics of a high-quality software product. They help teams
ensure all important quality aspects (from functionality to security) are
considered before release.

 Evolution: Quality models have evolved over time – ISO 9126 (1991) was an earlier
standard, and ISO 25010 (2011) built upon it, reflecting new priorities like security and
compatibility.

2.1- ISO/IEC 9126 Quality Model (Overview)

 Overview: ISO/IEC 9126 defines six main characteristics of software quality. This
standard (first issued 1991, updated 2001) was a foundational model for specifying and
measuring software quality.

 The 6 Quality Characteristics: Functionality, Reliability, Usability, Efficiency,


Maintainability, Portability. Each of these is further broken down into sub-
characteristics (for example, Functionality includes suitability, accuracy, etc.).

 Usage: ISO 9126 provides a framework for evaluating quality through internal metrics
(code quality), external metrics (runtime behavior), and quality-in-use metrics (user
perspective). This ensures quality is considered from developer, system, and end-user
viewpoints.
2.2- ISO/IEC 25010 Quality Model (Overview)

 Overview: ISO/IEC 25010 is the successor to 9126, published in 2011. It expands and
refines the quality model to address modern software needs.

 The 8 Quality Characteristics: Functional Suitability, Reliability, Usability, Performance


Efficiency, Compatibility, Security, Maintainability, Portability.
(Notably, Security and Compatibility were added compared to ISO 9126.)

 Notable Changes: “Functionality” is now Functional Suitability (with sub-characteristics


like completeness, correctness, appropriateness), and “Efficiency” is
termed Performance Efficiency. The model also explicitly
highlights Security (confidentiality, integrity, etc.) and Compatibility (co-existence,
interoperability) as first-class quality attributes.

 Quality in Use: ISO 25010 includes a separate Quality-in-Use model (effectiveness,


efficiency, satisfaction, etc. in actual use) and even Context coverage, recognizing that
software quality also depends on the usage context. (These ensure the product’s impact
on end-users is measured, not just the product itself.)

2.3- ISO 9126 vs. ISO 25010 – Key Differences

 Number of Characteristics: ISO 9126 had 6 quality characteristics, whereas ISO 25010
has 8, adding Security and Compatibility as new focus areas.

 Modernized Focus: ISO 25010 reflects modern software concerns – e.g. security and
interoperability are critical today, hence elevated. It also reorganized some concepts
(e.g., Suitability in 9126 became Functional Suitability with clearer sub-metrics).

 Quality in Use: The newer standard explicitly addresses quality-in-use and context (how
the software performs for users in real conditions), which ISO 9126 touched on but
25010 expands.

 Implications: Moving to ISO 25010 means evaluating software with a broader lens
(more characteristics) that cover current technology trends (like mobile, cloud, multi-
system integration). In practice, ISO 25010 provides a more comprehensive and updated
framework for quality evaluation, aligning QA with user satisfaction and emerging
concerns.
3- QA Planning Overview

 What is QA Planning? It’s the process of defining the testing strategy and
procedures for a project. A QA plan outlines what will be tested, how it will be
tested, who will test it, and when testing will occur.

 Why Plan? A well-defined QA plan ensures potential problems are identified early,
saving time and money by preventing late-stage defects. It provides a roadmap so that
testing is systematic and not ad-hoc.

 Deliverables: The outcome of QA planning is a Test Plan document that covers scope,
approach, resources, schedule, and metrics. This guides the QA team and informs
developers and management about the testing process.

3.1- QA Planning Steps (1/2)

 1. Gather Requirements: Thoroughly review requirements, design documents, and user


stories. Engage stakeholders to clarify expectations. Rationale: Understanding what the
software should do is the cornerstone of planning effective tests.

 2. Set Test Objectives: Define clear objectives for testing. Align them with business goals
and define success criteria for each (use SMART criteria: Specific, Measurable,
Achievable, Relevant, Time-bound). Example: “All critical user workflows must pass end-
to-end tests with no Severity-1 defects.”

 3. Define Scope: Delineate what’s in scope and out of scope for testing. Identify which
features, modules, configurations will be tested and which will not. This helps focus the
QA effort and manage stakeholder expectations (preventing surprise gaps or over-
extension).

3.2- QA Planning Steps (2/2)

 4. Allocate Resources: Determine who will do the testing (team size, roles), what
tools are needed, and set up test environments. Ensure testers have the necessary
training, and environments mimic production as closely as possible.

 5. Risk Assessment: Identify potential risks (tight deadlines, complex new technology,
etc.) and prioritize them. For each risk, plan mitigation strategies (contingency plans,
additional testing for high-risk areas, etc.). This proactive step prevents surprises during
testing.

 6. Schedule & Milestones: Establish the testing timeline – when test design is done,
when execution starts, and end dates. Include milestones like “Test Plan sign-off,” “Test
Case ready,” “Mid-way test execution review,” etc. This keeps the QA activity on track
and visible to the team.

is thorough and aligned with project goals.)

3.3- Test Plan Components

 Test Scope & Features to Test: Clearly list the features, modules, or requirements that
will be tested. Also mention explicitly any features not being tested (out-of-scope) to
avoid assumptions.

 Test Strategy & Approach: Describe the overall approach (functional testing,
performance testing, security testing, etc.). Define levels of testing (unit, integration,
system, UAT) and the types of testing (manual, automated, exploratory) to be performed
for this project.

 Roles and Responsibilities: Identify team members and their roles (e.g., QA lead, tester,
automation engineer, developer support, etc.). Include who will approve the test plan,
who will sign off on completion, and points of contact for defects.

 Environment & Tools: Specify the test environments (hardware, OS, test data setup) and
tools (test management tools, automation frameworks, defect tracking systems) that will
be used.

 Test Schedule: Outline the timeline for testing activities, including start/end dates for
test execution, any testing cycles (e.g., regression cycles), and milestones.

 Deliverables & Metrics: List what documents and reports will be produced (test cases,
test reports, defect reports, metrics dashboards). Define entry and exit criteria for
testing phases (e.g., “All critical defects fixed before UAT” as an exit criterion for system
testing).

3.4- QA Planning Best Practices


 Start Early (Shift-Left): Begin QA planning early in the project. Involve QA during
requirements and design phases so that test planning can uncover requirements gaps or
design inconsistencies sooner rather than later.

 Involve All Stakeholders: Collaborate with developers, product managers, and business
stakeholders in crafting the QA plan. This ensures the test plan aligns with business
needs and dev constraints, and everyone agrees on quality goals.

 Clear Communication: Make the QA plan accessible to the whole team. Communicate
the testing process and criteria. For example, ensure developers know how bugs will be
reported and what the definition of “done” (quality-wise) is.

 Be Adaptive: Treat the QA plan as a living document. Update it as requirements change


or new risks are identified. Avoid rigidity – be prepared to adjust test scope or strategy if
the project pivots (while communicating changes to stakeholders).

 Metrics-Driven Adjustments: Use early metrics (like test progress, defect counts) to

4- QA Metrics – Introduction

 What are QA Metrics? They are quantifiable measures used to assess the quality of the
software and effectiveness of the QA process. Metrics provide concrete data on aspects
like defect counts, test coverage, and efficiency.

 Why Metrics Matter: They enable data-driven decisions in the project. Rather than
guessing, teams use metrics to know if the product meets quality standards and if the
testing process is sufficient. For instance, a trend of decreasing defect density suggests
improving quality.

 Common QA Metrics: Metrics span product quality (e.g., number of defects, test
coverage), process quality (e.g., test execution speed, re-test rates), and project
progress (e.g., testing completed vs planned).

 Management & Engineers: For management, metrics show overall quality health and
readiness (e.g., open defects by severity). For engineers, metrics pinpoint areas of
weakness (e.g., a module with high defect density) and drive improvements.

4.1- Types of QA Metrics


 Product Quality Metrics: Measure attributes of the software product itself – its defects
and capabilities. (Ex: defect density, requirements coverage).

 Process Quality Metrics: Measure the effectiveness of the QA/testing process. They
reflect how well testing is being executed. (Ex: test case pass rate, mean time to repair
bugs).

 Project Metrics: Measure project management aspects of quality assurance. These


relate to project delivery, timelines, and cost. (Ex: test execution progress, time to
market, cost of quality).

 This classification helps in monitoring different perspectives of quality. For instance, you
may have a great process (high test pass rate) but the product quality could be poor
(high defect density) – so you need to watch all types.

4.2 - Examples of Product Quality Metrics

 Defect Density: Number of defects per size of software (e.g., per 1000 lines of code or
per function point). A lower defect density indicates higher code quality. It’s used to
identify problematic components (a module with high defect density needs attention).

 Test Coverage: Percentage of requirements or code covered by tests. For example, if 90


of 100 requirements have at least one test case, requirements coverage is 90%. Higher
coverage can mean fewer untested areas, though 100% coverage doesn’t always equal
0 bugs.

 Defect Leakage: Count of defects that escaped to production divided by total defects.
This metric shows how effective QA was at catching bugs before release. A low leakage
rate means QA caught most issues internally.

 Examples (from industry): Other product metrics include Defect Removal Efficiency
(DRE) – the percentage of defects removed before release, Severity Index – weighted
measure of defect severities, etc, all aiming to quantify the product’s quality level.

4.3- Examples of Process Quality Metrics

 Test Case Pass Rate: The ratio of test cases that passed to total executed in a cycle. If 95
out of 100 test cases passed, pass rate = 95%. This indicates the stability of the build
under test.
 Bug Reopen Rate: Percentage of defects that were thought fixed but had to be
reopened. A high reopen rate might indicate inadequate fixes or insufficient verification.
Keeping this low is a sign of effective debugging and validation.

 Mean Time to Repair (MTTR): Average time taken to fix a defect once it’s reported.
Shorter MTTR means the development team is quickly addressing issues – important for
fast iterations.

 Automation Coverage: Proportion of test cases (or test steps) that are automated.
Higher automation coverage (especially for regression tests) can improve process
efficiency and consistency.

 Process Efficiency: Metrics like Test Execution Productivity (test cases executed per
person-day) or Defect Detection Rate (defects found per day of testing) gauge how
efficient the QA process is, helping identify bottlenecks.

4.4- Examples of Project Metrics

 Test Execution Progress: Often tracked as a burn-down or burn-up chart of tests. E.g.,
“80% of planned test cases executed, 70% passed” by a certain date. It shows if testing is
on schedule or if there’s a backlog of unexecuted tests.

 Time to Market: The time taken to deliver the software (or a new release) to end-users.
While not solely a QA metric, delays in testing can affect this. QA efforts focus on not
compromising quality while meeting the timeline.

 Cost of Quality (CoQ): This includes the cost of all quality activities (testing, tools, QA
team) plus the cost of poor quality (like rework, defects in production). For example,
investing in more test automation may raise upfront cost of quality but can reduce cost
of poor quality by catching bugs early.

 Defect Trends: For project tracking, teams also look at trends like Open Defects Over
Time. Management monitors if the defect count is dropping as release approaches
(expected), and that no high-severity issues remain open.

balancing quality with time and budget.)

4.5- Best Practices in Using Metrics


 Select Relevant Metrics: Focus on a handful of metrics that matter for your project.
Don’t measure everything – choose metrics aligned with your quality goals. For instance,
an safety-critical app might focus on severity of defects and test coverage, whereas a
startup product might emphasize time to market and user-reported bugs.

 Define Metrics Clearly: Everyone should understand what each metric means and how
it’s calculated. Define them in the test plan (e.g., what counts as “a defect” for defect
density, how severity is ranked) to avoid confusion.

 Track Trends, Not Just Numbers: A single data point might not tell much – look at the
trend over time. e.g., Defect density dropping release over release indicates
improvement. Use graphs to visualize trends so both engineers and managers can see
progress or issues at a glance.

 Avoid Vanity Metrics: Ensure the metrics drive action. If a metric isn’t actionable (team
can’t change anything based on it) or doesn’t reflect quality (just “looks good”),
reconsider it. For example, 100% test case pass rate is meaningless if your test cases are
too shallow; instead, also monitor if critical user paths are covered.

 Regular Review: Discuss metrics in periodic meetings (e.g., weekly QA status). This
keeps the team informed and allows quick response if a metric shows an out-of-bound
value (like sudden spike in open bugs). Management appreciates seeing these to gauge
release readiness.

 Balance Quantitative with Qualitative: Numbers are important, but combine them with
qualitative insights (tester intuition, user feedback). For instance, a low defect count
might mean high quality or it might mean insufficient testing – tester analysis can tell
which.

5- Defect Management Overview

 Definition: Defect management is the process of identifying, tracking, and resolving


bugs or defects in software. It covers the defect’s journey from the moment it’s found to
its final resolution (closure).

 Defect Life Cycle: A defect goes through various states from discovery to closure. For
example, it might start as “New,” then move to “Assigned,” “Fixed,” “Verified,” and
“Closed.” (We will detail these stages next.) The life cycle imposes a structured
workflow so nothing falls through the cracks.
 Objectives: The goal is to ensure each reported issue is properly addressed – either
fixed or otherwise dispositioned (e.g., deferred). Defect management
provides visibility of the status of all known issues to the team and stakeholders. It
improves efficiency by coordinating the work on defects and prevents lost/forgotten
issues.

 Visibility & Communication: Using a consistent defect process allows developers,


testers, and managers to communicate clearly about bug status (e.g., everyone
understands what “Deferred” means). This shared understanding speeds up resolution
and decision-making on whether the product is ready to release.

5.1- Defect Lifecycle – Stages

 New: When a tester finds a new defect, it is logged as “New.” At this point, it’s a
candidate to be fixed – described with steps to reproduce, expected vs actual results,
etc.

 Assigned: The defect is triaged and assigned to a developer (or team) to fix. The project
lead or QA lead confirms it’s a valid issue and prioritizes it, then it moves into the
developer’s queue.

 Open (In Progress): The developer has started working on the fix. The bug is “Open” (or
“In Progress”). If for some reason the developer concludes it’s not a valid defect or not
urgent, they might move it to a different state (like “Rejected” or “Deferred” – more on
these later).

 Fixed: The developer has made a code change they believe resolves the issue, and marks
the defect as “Fixed”. The fix is then delivered to QA (in a build) for re-testing.

 Retest (or Pending Retest): Now the tester re-tests the application to verify the fix.
During this phase, the defect might be marked as “Retesting” or “Pending Retest”
(waiting for tester verification).

 Verified: The tester confirms that the defect is indeed fixed – the originally reported bug
no longer occurs. The defect status is updated to “Verified” (also sometimes called
“Resolved – Verified”).

 Closed: Finally, the defect is marked “Closed” by the QA lead or tester, indicating the
issue has been fully resolved and no further action is required. This is the end of that
defect’s life cycle.
 Reopened (exception): If in the Retest phase the issue is still present (the fix failed or
only partially worked), the tester moves the bug to “Reopened.” It then goes back to
the Open state and must be fixed again. The cycle then repeats from the development
stage.

5.2- Defect Tracking Systems

 Purpose of Tools: Given the potentially hundreds of defects in a project, specialized


tools (defect tracking systems) are used to record and manage them. These systems
provide a central repository where each bug is logged, prioritized, and tracked through
its life cycle.

 Popular Tools: Examples include JIRA, Bugzilla, Redmine, Trello, and others. JIRA is one
of the most widely used, offering real-time defect tracking, dashboards, and
customizable workflows. Open-source options like Bugzilla provide robust features for
smaller teams or those on a budget.

 Key Features: Good tracking tools allow attaching screenshots/logs, linking defects to
test cases or user stories, assigning owners, setting severity/priority, and notifying the
team of status changes. They also enable reports (e.g., number of open bugs by priority,
average resolution time).

 Integration: Modern defect trackers integrate with development tools – e.g., link issues
to source code commits or to CI/CD pipelines. This integration means when a developer
checks in a fix, it can update the defect status automatically. It creates traceability: you
can trace a customer-reported issue all the way to the code change that fixed it.

 Management Insight: For management, these tools provide visibility into project quality.
At any time, one can see how many defects are open, how many critical vs minor, and
thus gauge the risk of releasing. This helps in go/no-go decisions for releases.

5.3- Defect Triage & Prioritization

 Triage Meetings: Teams often hold regular “bug triage” meetings to review new and
open defects. In triage, bugs are sorted by impact and urgency. The team (QA,
development, product manager) decides on each defect: Should we fix it now or later?
Who will fix it? What priority and severity does it get?

 Severity vs Priority: It’s important to establish a clear severity/priority system. Severity =


how bad the bug’s impact is (e.g., crash vs minor UI glitch). Priority = how soon it should
be fixed (business urgency). For example, a minor typo might be low severity but could
be high priority if it’s on a landing page for a demo. A matrix or guideline helps avoid
debates.

 Handling Backlogs: If bugs pile up, triage helps keep the backlog in check. The team
might decide to defer low-priority bugs to a later release so that critical ones get fixed
first. Regularly reviewing and re-prioritizing ensures that at any time, the team is
working on the most important defects.

 Stakeholder Involvement: Involving product owners or clients in prioritization ensures


alignment with business needs. Sometimes what developers think is critical might not
be, and vice versa. Clear communication (using data like how often a bug occurs, or
impact on user reports) aids in making informed decisions.

 Outcome: Effective triage and prioritization mean high-severity and high-priority bugs
get resolved first, improving overall stability. It also provides transparency – everyone
knows why certain bugs are fixed immediately and others are scheduled for later.

6- Defect Prevention – Proactive QA

 Concept: Why just find bugs when you can prevent them? Defect prevention is about
taking actions during development to stop defects from occurring in the first place. It
complements testing by focusing on root causes of defects and addressing them early.

 Approach: Teams analyze past defects to identify common causes, then implement
practices to avoid those mistakes. It’s a strategy built on continuous learning and process
improvement. For example, if many bugs were due to ambiguous requirements, a
preventive action is to improve the requirements review process.

 Benefits: Preventing a defect is more efficient than fixing it after the fact. Early defect
detection or prevention leads to easier and cheaper fixes (issues caught in requirements
or design cost far less to resolve than those found after release). It also means higher
quality outcomes, since fewer defects escape into later stages.

 Mindset: This requires a cultural shift – developers and QA collaborate closely,


and quality is everyone’s responsibility from day one. Instead of QA being only the
gatekeeper at the end, QA practices (like reviews, static analysis) are embedded
throughout development.

 Examples of Prevention Focus: We’ll discuss specific techniques next (like code reviews,
TDD, etc.), but the overarching idea is to build quality into the product, not just test for
it at the end. Teams that excel in defect prevention often have lower bug counts and can
deliver faster because they aren’t slowed down by as many fixes.

6.1- Defect Prevention Techniques (1/2)

 Requirements Analysis: Many defects originate from unclear or wrong requirements. By


doing thorough requirement analysis and review, the team can clarify ambiguities and
correct issues in the specification phase. Ensuring requirements are well-understood
prevents building the wrong functionality. Techniques: Requirements workshops,
ambiguity reviews, using examples/user scenarios to clarify.

 Design & Code Reviews: Peer reviews and inspections of design documents and code
are powerful defect filters. Before code even runs, having developers inspect each
other’s code (or doing formal code inspections) can catch logical errors, design
inconsistencies, or deviation from standards. This often catches defects early, when they
are cheapest to fix.

 Defect Logging & Analysis: For any defects that do occur, ensure they are well-
documented and later analyzed. Keeping a defect log with details helps in understanding
patterns. Periodically analyze the defects to identify root causes. For example, if multiple
bugs are due to a specific API misuse, that’s a signal to improve how developers use that
API (through training or utility functions). This feedback loop prevents the same
mistakes from recurring.

 Root Cause Analysis: When a significant defect is found, perform a Root Cause Analysis
(RCA). Ask “why did this happen?” five times, if needed. Identify the process breakdown
(lack of code review? missing unit tests? miscommunication?). Once the root cause is
known, implement preventive measures. Example: If RCA finds that a bug slipped in due
to unclear code requirements, the preventive action could be to add a checklist item to
always have a requirements review with QA present.

 Process Improvements: Embed defect prevention into your process. This could mean
updating checklists, improving developer onboarding/training on common pitfalls, or
adding a step in the development workflow (e.g., every user story must have acceptance
tests defined before coding begins). Over time, these improvements lead to fewer
mistakes and a more mature development process.
6.2- Defect Prevention Techniques (2/2)

 Static Code Analysis: Use automated static analysis tools to scan code for known error
patterns, security vulnerabilities, and coding standard violations before the software
runs. These tools can catch things like null pointer dereferences, risky functions, or style
inconsistencies that could lead to defects. Integrate static analysis into the CI pipeline so
that code with certain warnings is fixed early.

 Pair Programming: Adopt pair programming in critical areas – two developers working
together at one workstation. One writes code, the other reviews in real-time. This
practice can significantly reduce defects because it’s like a constant code review as code
is written. It also spreads knowledge and enforces better coding habits, preventing
defects from sloppy or unreviewed code.

 Test-Driven Development (TDD): In TDD, developers write automated tests for a new
function before writing the code itself. Then write code to make those tests pass. This
ensures that the code meets the intended behavior from the start and that edge cases
are considered. TDD can prevent defects by catching them at the moment of coding – if
new code breaks a prewritten test, the developer knows immediately.

 Continuous Training: Keep the team’s skills sharp. Regular training sessions on secure
coding, common pitfalls, new testing tools, etc., can prevent defects caused by ignorance
or bad practices. For example, training developers on common security vulnerabilities
(OWASP Top 10) can prevent introduction of those issues. A knowledgeable team is less
likely to introduce defects.

steps are not missed. For instance, a checklist for code review might include “error
