Scenario:
The rise in syphilis cases among pregnant women presents significant health risks, particularly
due to the potential for congenital syphilis. This syphilis screening promotion intervention is
designed to increase awareness and uptake of syphilis screening among pregnant women in high-
risk areas outside Oklahoma. Your group will create a logic model to guide this intervention,
identifying resources, actions, and anticipated changes.
Objective:
Design a logic model with five components: Inputs, Activities, Outputs, Short-Term Impacts,
and Mid-Term Impacts. Use clear, concise explanations and logical connections to demonstrate
the flow from resources to expected changes.

Assignment Instructions
Provide your responses in main text format, following the structure below.

Logic Model Components


1. Inputs/Resources (4 or more)
1) Identify and list the key resources required to support the intervention. These can include
partnerships, educational materials, technology, funding, or community support.
2) If-Then Statement: Provide a statement explaining how these resources will enable the
planned activities.
Example: If we secure partnerships with local schools and receive grant funding, then we can
develop targeted educational workshops and materials.

2. Activities/Interventions (3 or more)
1) Describe the main actions your intervention will take to educate and engage the target
population. Think about activities that are appropriate and effective for reaching the intended
audience.

2) If-Then Statement: Use a statement to show how these activities will produce measurable
results (outputs).

3. Outputs (3 or more)
1) Outputs are the direct, measurable results of the activities, demonstrating that they were
conducted as planned. These should be specific indicators that the health intervention is reaching
the target audience.
2) If-Then Statement: Provide a statement illustrating how achieving these outputs will set
the stage for further impacts.

4. Impacts: Short-Term Effects (3 or more)


1) Short-term impacts are the initial changes in knowledge, attitudes, or awareness that result
from the outputs. These impacts represent the immediate reactions or understanding gained
by the target population.
2) If-Then Statement: Explain how these short-term impacts will contribute to achieving the
mid-term impacts.

5. Impacts: Mid-Term Effects (2 or more)


1) Mid-term impacts represent more stable changes in behavior or environment, showing
progress toward the intervention’s overall goals. Consider shifts in behaviors or conditions
that indicate sustainable change.
 Behavioral Objectives: For example, increased adoption of healthier eating habits among
teenagers.
 Environmental Objectives: For example, improved availability of healthy food options at
school.
2) If-Then Statement: Describe how these mid-term impacts will contribute to the overall
success of the intervention.
Example: If teenagers’ healthier food choices over time and increased availability of healthy
food options at school (mid-term impact) would lead to significant reduction in childhood
obesity (long-term outcome)

6. Context or External Conditions (Optional)

Consider any external conditions that could influence the intervention’s success, such as
socioeconomic factors, cultural norms, or policy changes. Although this section is optional, it
can add depth to your logic model by acknowledging external influences.

Reminder on Developing Logical Relationships
