CLIENT MEDIA RELEASE CONSENT FORM

I give permission to ISSofBC (Immigrant Services Society of BC) to take


photographs and/or video of myself, and/or to use photographs and/or
video which I willingly provide for the purpose of production and
dissemination in print and online channels including social media that
promote and further the goals of ISSofBC. I also give permission to ISSofBC
(Immigrant Services Society of BC) to use the photographs and/or videos of
myself after I am no longer a client of ISSofBC (Immigrant Services Society of
BC). Publications include those produced by ISSofBC, as well as those
produced by third parties with ISSofBC’s permission. I understand that I
may withdraw consent at any time.
