By signing this, I understand and agree to the following:
 I failed to register by the published deadline.
 Late add petitions are designed for situations involving extenuating circumstances
beyond my control, they are not simply an extension of the add deadline.
 Late add petitions without sufficient detail will be denied, and statements made on the
petition will require verification (Step 2).
 I must request instructor signature and explanation. I am aware the instructor is not
required to sign and/or approve this petition (Step 3).
 I must request Division Dean signature and approval. I am aware the Division Dean is not
required to sign and/or approve this petition (Step 4).
 The Admissions and Records Office is not required to approve this petition even if the
student explanation constitutes an extenuating circumstance and the petition is
supported by instructor and Division Dean (Step 5).
 Once a late add is processed it cannot be reversed and no refunds can be granted.
 If petition is approved by Admissions and Records (Step 5), all outstanding and related
registration fees are due immediately.
 If petition is approved by Admissions and Records (Step 5) and course pre-requisites have
not been cleared, the Late Add will not be processed. Pre-requisites must be cleared
prior to submission of the petition.
 I must submit this petition to Admissions & Records on or before the return deadline, as
stated on the top of this form (box on upper right hand side).
