Task: You are tasked to complete a template for a health and social care patient care plan. In this template you will show your
understanding of why each section is needed. You will fill out the template so that the reader can understand professional boundaries
and use of professional codes of conduct, as well as safeguarding policies and procedures that are needed to show good practice.
Understanding of key caring techniques will be shown throughout. Overall, you will demonstrate the importance of each area of a care
plan through showing the reasonings for each section.
A care plan will work to assess health and social care needs, and how you will support your service user. It specifies who will provide care, what
type of care is needed, and how the support will be given.
You will use the template of a care plan in this assignment to give evidence that you have learned how to support a service user in each section.
