E 4 HIKING AND COOK OUT

I, [Parent/Guardian Name], give my consent for my child, [Student’s Name],


to participate in the P.E 4 Recreational Activities organized by Bantayan
Southern Institute. This activity involves hiking and a cookout at Quarry, Big
Cross located in Kabangbang, Bantayan, Cebu.

I understand that while every precaution will be taken to ensure the safety
and well-being of the students, there are inherent risks associated with
outdoor activities. I acknowledge that neither the school nor its staff can be
held liable for any accidents or injuries that may occur during the course of
the activity.

I hereby authorize the school staff to seek medical attention for my child in
the event of an emergency. I also confirm that my child is physically capable
of participating in the mentioned activities and has no known medical
conditions that would prohibit their involvement.

Furthermore, I agree to ensure that my child adheres to all safety guidelines


and instructions provided by the school staff throughout the duration of the
activity.
