Pre-assessment of Individual Student Readiness
Explain in one paragraph, when the pre-assessment would be administered in relation to the lesson – eg: at the end of the previous
lesson; with structured observations collected over the previous week, etc. Remember that this should be a pre-assessment of readiness
specifically related to the tiered lesson, and not a pre-assessment for the whole unit, so be clear about how, specifically this will help you
to make decisions about differentiation based on individual readiness in relation to the objectives. Include your pre-made pre-assessment
in the appendices.

Explain in one paragraph, the allocations of students to three distinct readiness groups. Eg: students who could correctly answer all
questions would be allocated to the high readiness group. Remember to be quite specific with regards to the allocations using your
questions and possible answers to those questions. You may use a checklist or a rubric and include this in your appendices, and refer to
this in your paragraph. Use the terms: High readiness group, Middle readiness group and Low readiness group. You may create names
for the groups to avoid negative effects on student motivation and mindset.

Lesson Plan Lesson length: Lesson number: (eg. 3 in a unit of 6)

Lesson Closure/ Check for understanding

How will you know that students have achieved your lesson objectives? Include both a formal (exit card, work sample etc) assessment
and an informal (class discussion, show and tell etc) assessment. Can create formal assessment and refer to it in your appendices.
Explanation
Provide 2 – 3 paragraphs expanding on your lesson plan notes to clearly explain how your lesson is an example of a tiered lesson
designed to address differences in students’ readiness, and what aspect/s of readiness you are aiming to address. This explanation should
demonstrate your understanding of differentiation by readiness, supported by the topic readiness and materials.
