I.

TRAVEL ABROAD (PERSONAL TRAVEL) (3 Copies each)


All the following documentary requirements must be in three (3) copies and must be endorsed by the school head to the Division Office 20 days before the
departure date and received at the Regional Office ten (10) days before the departure date.
1. 1st Endorsement from School Principal
2. Letter of intent to travel abroad (Address to Schools Division Superintendent)
3. Form 6 (with Division HR signatory in Field 7.A next to AO’s/ADAS’ signatory)
4. Division Clearance
5. Certificate as to no pending administrative case (will be requested at Administrative Office - DO)
6. Clearance for Provident Loan (Accounting – DO) (If with existing loan, promissory note is desired)
7. Certificate of Dispense Conformed with by teacher who will take over the class during his/her leave of absence.
8. Certificate of Acceptance
9. Medical Certificate (for consultation or treatment of ailment or sickness)
10. *Special Order designating an Officer-in-Charge (For School Heads) duly signed by SDS
* For Travel Abroad during Vacation (Teachers Only): Form 6 and Cert of Dispense are not required

II. TRAVEL ABROAD (OFFICIAL TRAVEL) (3 Copies each)


1. 1st Endorsement from School Principal
2. Written justification as required in paragraph IV.A.2 of DepEd Order 043, s.2022
3. Endorsement Letter from Division (HR)
4. Division Clearance
5. Administrative Clearance (will be requested at Administrative Office - DO)
6. Form 6 (with Division HR - signatory in Field 7.A next to AO’s signatory)
7. Certificate of Dispense
8. Certificate of Acceptance
9. Clearance for Provident Loan (Accounting – DO)
10. Approved Activity Request and Work and Financial Plan indicating that funds are earmarked for travel expenses to be incurred
11. Itinerary of Travel
12. *Special Order designating an Officer-in-Charge (For School Heads) duly signed by SDS
13. Locator Slip
14. Travel Authority for Official Travel

Processing Travel Abroad Leave*


1. Endorsement Letter from the Schools Division Superintendent
2. Letter request of the applicant (starting justifiable reasons)
3. Clearance as to money and property accountabilities & responsibilities (noted by SDS)
4. Certification that he/she has no pending administrative case (noted by SDS)
5. CSC Form 6 with HR-Division signatory
6. Certification that his/her services can be dispensed with during vacation leave of absence and explicitly or clearly indicate the name of teacher/ principal who will
take over of his/her class during the duration of his/her absence.
7. Certification that he/she did not avail provident loan
8. Medical Certificate (for sick leave) and abstract form officially accredited hospital
9. Certificate of acceptance by the concerned employees who will take charge of the duties and functions of the applicant while on vacation leave of absence,
noted by the immediate supervisor or head of the school.

Note: Applicant MUST file Return to Duty upon returning or before returning to service.
