7 POINTS TO REMEMBER FOR TEACHER REFLECTION FORMS (TRF)

1. When taking the teacher reflection, takers are not allowed to bring
their cellphones. Only TRF and ballpen are found on the armed chair/
desk /table. Other things must be placed on the table found in front
of the classroom under the care and responsibility of the TRF
proctor.
2. After the TRF distribution, please let the teacher applicants peruse
each page seeing to it that each set consists of 5 pages and the first
4 pages have the questions based on objective 6-9. The last page (5)
is the Inter-Evaluator Agreement (IEA) form which is the rating
sheet and this is now the checker’s concern with the code that must
be reflected by the applicant. Should there be missing pages or
pages which are torn/ stained by coffee, etc. buffer can be used
however, proctors must justify it with incidental report.
3. Each page is stamped with fresh signature, if the stamped page has
no fresh signature, it could also have the facsimile signature of Dr.
Carmelita A. Alcala, our OIC-CID Chief.
4. Depending on proctor’s convenience, you can have the numbering
before or after the test. See to it that the number found at the
control sheet with codes pasted in the brown envelop is the same
number written on the upper left corner of the TRF (front page only)
So, during retrieval, all we need to check are the numbers and not
anymore the codes because it is implied that at the district level you
have very well rechecked the veracity of the teacher applicant
codes.
5. As to the number of hours or minutes in taking the test, please
remember what Sir Romel Victor A. Villahermosa, our ASDS oriented
at Hotel Essencia.
6. TRF coordinators and proctors should see that such activity must be
treated with safety and confidentiality. Only PSDSs, TRF
Coordinators and Proctors can touch the TRFs.
7. Upon retrieval of TRF next week, please proceed to the Personnel
Office and look for Merriel Segismar or Christin Camacho to accept
them. Be sure to sign the retrieval sheet before leaving or in my IP
demoteaching monitoring next week you may take the chance to let
me receive your TRFs. I can drop by, just pm me. Hereunder is my
schedule of IP demo-teaching monitoring:
