Why did you request an AAC evaluation?

Well, Jordan was diagnosed with cerebral palsy in 2010, which is


typically not a progressive disease, but what we were finding is
that she was progressing into having difficulties walking, talking,
and prior to the evaluation a couple of years ago, she was able to
speak two- to three-word sentences, and that declined, and we

were wondering what was going on, so we had some genetic


testing done and found out that she does not have cerebral palsy;
she has what's called spastic paraplegia, which is a progressive
disease, and it's... Her particular mutation is very rare, and there's
not a lot of literature on it. So this is one of the reasons why we
wanted to be able to support her and allow her to communicate
better with us and her friends.

Describe Jordan's current therapies.

Currently, she's getting all the therapies at school, your speech


therapy, physical therapy, occupational therapy. Outside private
therapies, we are getting physical therapy, occupational therapy,
she does a hippotherapy, which is the therapy on the horse. And
she also has the swimming once a week, which she's amazing at.

Describe Jordan's previous therapies.

Therapies in the past. Well, she started out with pretty much all the
same that I just spoke, and she's continued that throughout her
entire life really since about six months old. The only therapy we're
not doing now is the developmental therapy.

What are your goals for your child?

I want her to be a happy girl. I want her to be happy. I want her to


be free to communicate however she can and not be frustrated
and just to be able to sit on this couch with her family and tell us
exactly what she needs without us getting that look of frustration
and making her upset because we can't figure out what she wants.

So, earlier in her years when she was diagnosed with cerebral
palsy, she was making some great progress because we were
working toward her walking and she was using a walker. She had a
surgery called SDR, which is selective dorsal rhizotomy, which
releases the spasticity in her legs. And she was a rockstar, she was
doing great, making great strides, she was speaking more, she was
walking and really making great progress. And then two years after
that, she started to decline. And we lost a lot of the speech, she
wasn't able to communicate as well. Her upper body was more
affected, where before it was just her lower body. So... Jordan
works very hard, so the progress, she works as hard as she can to
strive in school and do very well in school. But we are seeing a
decline in her ability.

Developmental

So she gets around at school through a her mobile wheelchair,


which comes every day, and the bus picks her up and takes her to
school in that. At home, there are several ways we get around in
our home. So she does have the motorized wheelchair that we
sometimes bring in the home. She also has a medical stroller,
which we use when we're out traveling, and she has a trike bicycle,
which she absolutely loves.

So, Jordan's lost a little bit of her independence in the last few
years, but I can, that one thing that she does do very well
independently is swim. I mean, she does have some supportive
devices when she goes in the water, but she absolutely loves it.
And she's like a little fish. She does that very well by herself.

What do you need to help Jordan do?

The kind of things we need to help her to do would be, we have to


bathe her, we have to dress her, we feed her every day. So she's
lost the ability to do some of those things.

She is, she's taking what's called Lamictal, and she has what's
called absence seizure disorder, and they're very difficult to detect.
It's more of a gazing off, daydreaming type of seizure. So the
medication we're taking has probably been the last four years.

Does Jordan have any issues with eating and drinking?

Jordan has declined a bit in her eating and drinking. We've had
several swallow studies to ensure that there's no aspiration issues.
Thank goodness there has not been. But we have to take it slow
with Jordan when she's eating softer foods, drinking, just to make
sure she's sitting in the right position and she's getting enough of
that nutrition, so.

Well, this communication device is going to be really key for her to


communicate. She does have a lot to tell us, and she is a very
social, happy child, and we want to make sure that we're keeping
her feeling confident and wanting to be able to tell everyone what

Describe what Jordan understands.

Jordan understands everything. She has the ability to really, she's


actually very intuitive with what people are saying, and she can
understand everything. She just has a hard time communicating it
and that's one of the reasons why we are wanting to do this.

There's a lot of pointing, there's a lot of just noises to let us know


that she needs something. A lot of times, we have to pick her up
and she has to take us over to wherever it is that she needs
something so she can pull it off a shelf or get it out of the
refrigerator, and Jordan has a way of getting her point across.

Describe any strategies you use to communicate with Jordan.

We ask a lot of questions. The way we communicate with her is we


just, we use the room, we ask her if there's anything in the
particular room that she needs. We take her to that room for her
to show us and we, we try to, because she understands everything
that we say, we just try to do our best and try to identify it. And I
have to say it's very frustrating sometimes because sometimes we
just don't know what she needs.

Why are you wanting an SGD for your child?

I think that it's going to be something that she is going to thrive


with; I mean, she's going to be able to talk with her friends.
Typically what happens at school is, as you know for a third grader,
they move very quickly, so by the time Jordan explains what she
needs without a device, the kids are on to something else. So this
will really allow her to communicate with some of her very good
friends in school, her teachers. And they're really looking forward
to getting it because they do struggle every day with what she
needs and probably way more so than what we do. So that's one of
the reasons why we want to make sure that the school has the

ability to find out what she needs, and we also want to be able to
use that with our our family. And it's just something that we need
to be successful in knowing what she desires in life.

I am, we are, we're committed to learning the device because I


think it's critical for Jordan, and if we understand it, we can help
her through her process in learning it. So absolutely, we are very

Very similar to what I just mentioned, the strategies that she uses
to communicate would be a lot of pointing. She has the ability to
yell out fairly loud, especially if she is really needing something like

What are the current limitations with how you communicate with
her?

Some of the limitations would be, we don't have the device, so that
in itself is difficult because Jordan has a lot to say, and when she
uses this device, I can tell that she is really able to express how
she's feeling, what mood she's in, what she likes, what she doesn't
like. And right now, we don't have that device, so we're limited, and
at some point some days, I feel like I don't even know my own
daughter, like I don't know what she needs. And that's a limitation
for us.

She loves swimming, and she loves to ride horses. She's motivated
by music, and she loves to dance, and she likes to tell jokes. So she
loves to swim and she loves movies. She loves to travel. And she is

And loves being around family, and she's just she's just loves being
around fun family events and loves music and dancing.

arms, the first thing she does is kick in her lower back into
extension. With her reach being limited and with reduced
accuracy, it will be very challenging for her to access a device with
her hands. Additionally, I have had limited success with her being
consistent with a switch.

speech output. She was not making progress with producing
intelligible speech and actually started to decline and say less and

When I was working with her, she followed functional directions


when she was physically able to do so. She demonstrated
understanding of everyday conversation by laughing appropriately
at humor. She was informally assessed and showed understanding
of basic concepts such as location words, categories, size, shape,
colors, relationships between similar words, and a basic
understanding of synonyms and antonyms. Jordan understands
significantly more than she is able to communicate. Expressively,
Jordan communicates primarily through facial expressions and
AAC. Initially, she was saying some functional phrases verbally, but
her verbal skills have progressively declined. She has previous
diagnoses of dysarthria as well as mixed expressive/receptive
language disorder.
