OT home assessment 11/03/10. Modifications carried out including handrails in

District Nurse Visits twice daily for:
Assisting with transferring in/out of bed and grooming/hygiene needs
Monitoring medications
Assessing patient’s coping ability at home (including observation of wife’s
health)
Re-refer to hospital if necessary
WRITING TASK

referral to the district nurse at the District Nursing Centre, Lake Park, who will be
attending to Mr. O'Connor after his discharge.

Re: Mr Andrew O’ Connor, 74-year-old
I am writing to refer Mr Andrew O'Connor, who suffers from right-sided hemiplegia
and requires assistance from your team twice daily, as he is scheduled to be
discharged within three days.
On 02 December 2009, Mr O’Connor got admitted following a Left Cerebrovascular
Accident. During his stay, he has availed intensive physiotherapy and occupational
therapy. However, he is showing slow progress and still requires assistance in
dressing, toileting as well as getting in and out of the bed. Presently, he can feed
himself and mobilise slowly with a frame.
A family meeting was held to discuss the nursing home placement, but it was
declined. On this occasion, the occupational therapy team assessed Mr O’Connor’s
residence and made necessary modifications, including handrails in bathroom
and toilet as well as bed rails.
Upon discharge, Mr O’Connor will attend the hospital three times a week. He
requires your assistance with transfers, grooming and hygiene needs. His
medication compliance and overall coping at home must be monitored. Please
check on his wife Mary’s health condition also, as she has angina.
Kindly refer the doctor’s notes for his medication chart and medical history.
