Biostatistics One-Year Master’s Core Courses: Please list all the Biostatistics courses that have been, or will be taken, or
waived. Indicate the semester and year (e.g. F2020, S2021, SU2022, etc.) during which each of the courses was or will be

Note that a maximum of half your total credits per semester can be cross-registered courses without special
permission, and a maximum of half of your required 42.5 credits can be cross-registered courses. This is a School

Epidemiology Requirement: The School of Public Health requires that Master’s students must successfully pass one
Epidemiology course. The Department requires that EPI 201 be taken to satisfy this requirement. Ordinarily, this is taken
during the first semester in residence. Indicate the year during which you have/will have taken EPI 201.

Research Ethics Requirement: Research Ethics Requirement: See section 2.7.5 in handbook. Indicate the year
during which you have taken or received a certificate for the Research Ethics course or its approved
equivalent.

Culminating Experience Requirement: SM1 students must write up a summary of a culminating research experience that
has been performed beyond standard course work. The due date for this degree requirement is firm, and falls on the 3rd
Friday in April. More details are included in the BIO-SM Graduate Student Handbook in Section 2.13. Indicate the date
(month / year / ) by which you will submit your culminating research experience write up.

Note: Students must also satisfy the minimum number of ordinal credits required and maintain a cumulative average of
2.70 or above.
