JOB ORDER CONTRACT – 001


( SCHOOL UTILITY )

This contract is executed by and between the Republic of the Philippines through Dep Ed-Division of
Agusan del Sur Officer –in-charge Schools Division Superintendent and JENNIFER L. ALFECHE who
is a school utility and a resident of Purok -1, San Lorenzo ,Prosperidad, Agusan del Sur hereto after referred
as the job order employee.

1. That I , JAY E. LAYNO and JENNIFER L. ALFECHE do hereby agree to enter into a contract of
work under the following conditions on the job herewith described.

a. Perform duties and responsibilities in school as utility.


b. Assist the pupils / visitors in/ out of the school.
c. Submit Accomplishment Report
d. Perform other related task as the need arises.

2. That the job order employee agrees to devote his experience and skill covering the period from
JANUARY 1, 2024 to Dec. 30, 2024 including Saturdays and Sundays and Holidays (Day duties) with a
monthly rate of THREE THOUSAND PESOS only ( 3,000.00);

3. That the Job Contract may be renewed by subsequent agreement between the parties herein or
terminated for cause at the option of the DEP-ED – Officer-in-charge, School head/ or government Job Order
employee may also terminate this CONTRACT upon written notification to be submitted fifteen ( 15 )
days prior to the termination of this contract and after submission of a valid clearance from money and
property accountabilities;

4. That to record and / or document the actual attendance , the Job Order employee shall sign on the
logbook on a daily basis and reflect to the Form 48 to be certified by the School Principal./OIC-School
Head.

2024 at DEP ED – San Lorenzo Elementary School, San Lorenzo ,Prosperidad, Agusan del Sur.

GENERAL CONDITIONS OF PAKYAW AGREEMENT(LABOR ONLY)

1. All works called for under this agreement shall be enformed in accordance with the plans
and specifications set at the office of school Head, Azpetia ES. Any deviation from the plans
and specifications must be ordered in writing by the School of Azpetia through the School
Planning Committee chairman.

2. The Works shall be performed using labor based method under the supervision of the
Insfrastructure Chairman Committee.

3. The Pakyaw Group shall employ unskilled workers from school where the project is located;
seme – skilled workers from the municipality where the project is located and skilled
workers from the province which has jurisdiction over the project.

4. The school planning Committee chairman mat stop or suspend the contracted works any
time deems necessary by informing the Pakyaw Group writing. Corresponding payment shall
be made based on the amount of work accomplished.

5. The quantities and amount specified in the schedule of works were based on the quantities
and amount in the contract duly agreed and signed by both parties. Thus payment would be
made based on contract actual completion of works and acceptance by the implementing
agency.

6. The Pakyaw Group shall works atleast six (6) days a week including public holidays or as
scheduled by the school until the contracted works are completed and acceptance.

7. The Pakyaw Group shall take responsible and precaution to prevent misconduct among the
members. The Infrastructure Chairman Committee may require the Pakyaw Group to
remove from work any member who commits conduct or incompetency during the actual
performance of work.

8. The Pakyaw Group must have a minimum of 4 and a maximum of 6 workers unless directed
by the school planning Chairman Committee.

9. The implementing agency shall not be held liable for any accident, injuries or untoward
accidents that maybe encountered by any member of the Pakyaw Group during the
performance of the contract.

10. This Pakyaw Contract creates no employer – employee relationship between the
implementing agency or the government and the Pakyaw Group or any of its
workers/members

This contract is executed by and between the Republic of the Philippines through Dep
Ed-Division of Agusan del Sur Officer –in-charge Schools Division Superintendent and
MR MARIO A. ADAPTAR who is a security guard and a resident of Purok -1,
Azpetia ,Prosperidad, Agusan del Sur hereto after referred as the job order employee.

1. That I , APOLONIO A.ALIBANGBANG and MR. MARIO A.ADAPTAR do


hereby agree to enter into a contract of work under the following conditions on the job
herewith described.

a. Perform post duties and responsibilities in school as a security guard.


b. Conduct roving inspection to the area of my responsibilities
c. Submit Accomplishment Report
d. Perform other related task as the need arises.

2. That the job order employee agrees to devote his experience and skill covering the
period from January 2019 to March , 2019 including Saturdays and Sundays and
Holidays ( Night duties) with a monthly rate of ONE THOUSAND TWO HUNDRED
SEVENTY PESOS ONLY ( 1,270.00 );

3. That the Job Contract may be renewed by subsequent agreement between the parties
herein or terminated for cause at the option of the DEP-ED – Officer-in-charge, HT-II / or
government Job Order employee may also terminate this CONTRACT upon written
notification to be submitted fifteen ( 15 ) days prior to the termination of this contract
and after submission of a valid clearance from money and property accountabilities;

4. That to record and / or document the actual attendance , the Job Order employee
shall sign on the logbook on a daily basis and reflect to the Form 48 to be certified by
the School Head.

2. That the job order employee agrees to devote his experience and skill covering the
period from August 1, to Oct. 31, 2019 including Saturdays and Sundays and Holidays
(Day duties) with a monthly rate of ONE THOUSAND five HUNDRED PESOS only
( 1,500.00);

3. That the Job Contract may be renewed by subsequent agreement between the parties
herein or terminated for cause at the option of the DEP-ED – Officer-in-charge, School
head/ or government Job Order employee may also terminate this CONTRACT upon
written notification to be submitted fifteen ( 15 ) days prior to the termination of this
contract and after submission of a valid clearance from money and property
accountabilities;

4. That to record and / or document the actual attendance , the Job Order employee

Magsaysay ,Prosperidad, Agusan del Sur hereto after referred as the job order employee.
1. That I , APOLONIO A.ALIBANGBANG and ROGER T. PANGAPALAN do hereby
agree to enter into a contract of work under the following conditions on the job herewith
described.
a. Perform duties and responsibilities as labor worker .
b. The works shall be performed using labor based method under the
supervision of the school planning chairman or school head.
a. The school planning committee chairman shall stop or suspend the
contracted works any time if not following the specific job description or program
of works ( POW )
b. The SPT chairman or School Head has the power to remove from works any
member of the group who commits misconduct or incompetency during the actual
performance of works.
c. The implementing agency/ school shall not be liable for any accident ,injuries or
untoward incident that maybe encountered by any member of the pakyaw group
during the performance of the contract .
d. The group shall receive ( Php 3,000 ) Three Thousand pesos for labor .
2. That the Job Contract may be renewed by subsequent agreement between the parties
herein or terminated for cause at the option of the DEP-ED – Officer-in-charge, School
head/ or government Job Order employee may also terminate this CONTRACT upon
written notification to be submitted fifteen ( 20 ) days prior to the termination of this
contract and after submission of a valid clearance from money and property
accountabilities;
3. That to record and / or document the actual attendance , the Job Order employee
shall sign on the logbook on a daily basis and reflect to the Form 48 to be certified by
the School Head.
