RISK ASSESSMENT INFORMATION
Potential Hazard(s)
It is the operator’s responsibility to ensure that the worksite on which this equipment is to be used is
equipped with the appropriate fire protection devices as per Australian Standard AS 2444-2001 Portable
Fire Extinguishers and Fire Blankets.

Crush injury

Risk Level High

Control Measures Keep unauthorised persons away from site.

Be aware of all traffic, both pedestrian and vehicular, in the work vicinity.

Do not operate machinery without the use of a seatbelt.

Ensure persons are clear of reversing machinery.

Ensure warning devices such as flashing light and reversing beeper are functional.

Use safe lifting techniques – keep load as close as possible to the attachment
carriage, assess the weight and tip point of the load before raising forks. Limit the
load to the rated load capacity as shown on the load chart.

Observe maximum lift height and mast tilt.

Ensure loads are centred & secure.

Do not lift loads over persons.

When travelling with load, keep the load as low as reasonably practicable.

Move slowly and cautiously when the load is raised and/or visibility hampered by
load.

Avoid sharp turns and heavy braking, and ensure tyres pressures are at the
recommended Kpa.

Check height levels of doorways and overhead structures. Ensure load is at a


suitable height and proceed slowly and cautiously.

Check ground condition and slopes and remove all foreign objects. Keep level at all
times. Never operate on slopes exceeding 15. Do not travel across slopes or roll
over may occur. Always keep attachments and loads as close to ground as possible
and travel with heaviest end uphill and avoid turning on slopes.

Chock wheels when not in use.

Do not allow unlicensed and/or inexperienced persons to operate machinery. Do


not allow persons to sit or ride on the machine.

Explosion or fire from fuel vapours

Control Measures Stop engine before refuelling. Do not refuel while hot.

Carry out visual checks for fuel leaks and repair before operation.

Observe warning label on engine.

Keep flammables away from the engine.

Do not smoke while operating, servicing or refuelling this machine.

Use in well ventilated area.

Injury from contact with above ground services

Control Measures Check above ground for overhead power, branches, beams and structures.

Be alert – look up!

Do not raise forks within 10m of above ground services.

Employ the use of an experienced ‘spotter’ when working under or around


overhead power.

Exposure to ultraviolet radiation if working outdoors

Control Measures Wear sunscreen, hat, long sleeves and long pants.

Injury or ill health due to exposure to exhaust or fuel fumes

Control Measures Do not operate in confined space. Use in well ventilated area, preferably with
fume extraction ventilation. Avoid breathing or coming into contact with fuel.

Control Measures Ensure hands, feet and loose clothing are kept clear from moving parts of the
roller. Do not remove guards or covers.

Engine seizure/Fuel spillage

Risk Level Low

Control Measures Do not use on inclines of more than 15. Ensure equipment is operated on a
stable, level surface. If equipment is operated on a slope, engine seizure may occur
due to improper lubrication, even with max. oil level and functioning oil protection
system. Fuel spillage may also occur.
