Job Aid 1: Login and Change of Password and Loging Out

Login In
1. Launch Google Chrome or
Firefox web browser

2. Type web for CQI Database:

3. Enter Username and


Password

4. Click Sign in

Changing the Password

1. Click on Profile Menu

2. Click on Account

3. Enter the Old password, `


New password and repeat

4. Click UPDATE PASSWORD `

Login Out

1. Click on Profile Menu

2. Click on Logout

Job Aid 2: Creating and Editing a QI Project

Data Entry: QI Project


Information
1. click on Data Entry from
the top menu
2. Select a Health Facility
from the Organisation
Unit; i.e. Gulu RRH
3. Select QI Project Tracker
from program

NOTE: This will display the QI


projects previously added to
selected organization unit

4. Click Add

5. Enter the relevant details


for the project and click
Save Project

6. Click Cancel if you don’t


want to save the project

NOTE: You can add a new


indicator for the selected
Program Area by selecting ’Add
new indicator’ under the
options for Indicator for the
objective

7. Edit the QI Project by


navigating to the end of the
record under Data Entry

8. Clicking on Edit
Data Entry: Details

Note: Click on Details to


capture additional project
details: Change worksheet,
Run Chart and Summary of
Observations

Changes Worksheet

1. Select the Changes


Worksheet Tab

2. Click Add Changes


Worksheet

3. Add the details

4. Click Save

5. Navigate to the end of the


record to Edit the record

Run Chart and Annotation


1. Select the Run Chart and
Annotation Tab

2. Click Add Review

3. Add the Date, Numerator


and Denominator.
4. Repeat Step 4 until all the
values are captured

Summary of Observations
1. Select the Summary of
Observations Tab

2. Capture the required


details

3. Click Save to complete the


capture of the QI Project
details

Note: Click Mark Project As


Complete when the project is
completed
