Guidance:
• Read the Performance Management Policy before you start using this template.
• Use your Job Description (JD) to ensure you are clear about your responsibilities and the required competencies for
your job.
• Based on the JD, set between three and five SMART (specific, measurable, achievable, relevant, and time-related)
