Study the Reading on Portfolios from the Glossary of Education Reform. Based on your reading,
reflect on the following questions and jot down your answers in your Study Notebook:
1. How will your Portfolio help you in tracking the progress of your teaching practice in your
School’s LDM?
Portfolio will be a great help for teachers to keep track of the progress even tracking
areas to improve.
2. What evidence from the previous modules will help capture the progress of your teaching
practice?
Using the individual development plan from the previous module, we will be able to
capture the progress even tracking the areas to improve.

Familiarize yourself with the List of Suggested Evidence Form. Is the list of evidence enough to
capture the progress of your teaching practice? What other evidence can you think of that is
relevant to the LDM adopted by your School? Write down your answers and other insight in
your Study Notebook.
Yes, through these evidences I will be able to apply these in my teaching career and
monitor the progress of my pupils. Learning Activity Sheets (LAS) and Self-Learning Modules
(SLM) with MELCs that are unpacked or combined originally created by the teachers to suit
the needs of their pupils can also be used evidences. Documented outputs of pupils (portfolio)
highlighting the effectiveness of a particular learning modality and feedback forms or survey
forms from parents may also serve as evidences.

Discuss and share your thoughts on the Rubric in your LAC Session. Present your steps to make
your outputs and Portfolio responsive to the evaluation criteria and indicators. Take note of
your colleagues’ other insights as well.
I always make sure that I am well guided by the criteria in the evaluation rubric. This will
be a guide of knowing if I am still on a right track. I will consult/brainstorm with my colleagues
to continuously share my ideas to them and at the same time learn from them. I will also make
sure to properly document my outputs and follow the timeline set. Also, I will try to get
feedbacks from parents and other stakeholders regarding the effectiveness of the modalities
that I will be using in my delivery of instruction.
As a final reminder, make sure that you are able to collect the evidence of your teaching
practice as you have listed, and organize these into a Portfolio. At the end of the year, you will
submit your Portfolio to your LAC Leader, who will announce the specific date and manner of
submission.
