• 4 Passport size photographs in red/white background (2 additional postcard size


photographs in case of employees being covered under ESIC)
• Copies of all year Mark sheets and Degree certificates

Passport/ Utility bills/Aadhar card/ Govt. Bank Passbook/Affidavit
• Acceptance of Resignation/Relieving letter of last two employers
